0bjective: To compare four reference standards when evaluating a method of screening for referable diabetic retinopathy.We calculate sensitivity,specificity and kappa value of every method and screen a method of fast,high-performance detecting DR.Method: Diabetes clinics at Tongren Hospital were used in a prospective study of 108 people withâ… type orâ…¡type diabetes.Every diabetic undertake to exiamine eyesight.we adapt international standand visual actity chart to check out actity of vision in 5 meters and receive the results of one field colour fundus mydriatic photography, two field colour fundus mydriatic photography , six field colour fundus mydriatic photography and fundus fluorescein angiography ( FFA ) . Patients were selected from those attending clinics when the ophthalmologist and ophthalmic photographer were able to attend. The main outcome measures were the detection of referable diabetic retinopathy as defined new DR graded by the adaptation of American Association of Ophthalmoloogy.Then we carry out statistics contrast study of one field and six field colour fundus mydriatic photography,two field and six field colour fundus mydriatic photography,one field and FFA,two field and FFA,six field and FFA.Results: In comparison with six field photography,one field mydriatic digital photography gave a sensitivity of 95.16%, a specificity of 100.00%, and a kappa statistic of 96.50%. Two field mydriatic digital photography gave a sensitivity of 100.00%, specificity of 98.39%, and a kappa statistic of 98.80. In comparison with fundus fluorescein angiography,one field mydriatic digital photography gave a sensitivity of 72.46%, a specificity of 93.01%. Two field mydriatic digital photography gave a sensitivity of 73.9%, specificity of 93.01%.Conclusion: One field and six field colour fundus mydriatic,two field and six field colour fundus mydriatic contrast consequence show that one field colour fundus mydriatic photography and two field is a highly sensitivity,specificity and most perfect kappa value oppositting to six field colour fundus mydriatic ,difference is not very.We compysite from clinical point of view,high performance,shortcut,convenient and vanlance cheap, one field digital mydriatic photograph may be useful as a method of screening for diabetic retinopathy.
