Objectives To investigated the immune status of the chronic renal allograft dysfunction (CAD) patients, summarized the experiences of renal retransplantation. Methods Serum PRA was detected in CAD patients, patients experienced early graft loss, end-stage renal disease (ERSD) patients and recipients with long-term graft survival. Clinical data of CAD patients who underwent retransplantation was analyzed, and studied the percentage of CD3~+, CD28~+, CD25~+ and CD3~+CD25~+T-cell in peripheral blood. Results The PRA positive rate of CAD patients was higher than ERSD patients, while the PRA highly sensitized rate was not higher and pure anti-HLA class II antibodies positive was common. The general state of health of CAD patients was worse, and the incidences of postoperative complications were higher. The relative number of CD28~+T-cell was less in CAD patients before retransplantation. There were no difference in the incidence of acute rejection and mean serum creatinine level, neither were 1-year patient and graft survival rates. Conclusions If PRA-I class was negative, the immune status of CAD recipients would be closed to or lower than the first renal transplant recipients. Kidney retransplantation could yield desirable outcomes, if cautious selection, rational preparation, and reasonable use of immunosuppression therapy protocols were performed for CAD patients.
