Objective: Transcranial color-coded real-time sonography (TCCS) is a type of novel, safe and woundless ultrasonic inspection, which can not only display brain parenchyma and vascular blood flow in the same time, but also measure the flow velocity of BA precisely. In this study, the value of TCCS in diagnosis and classification in acute cerebral infarction was evaluated, and the relativity between atherosclerosis plaque in carotid artery and cerebral infarction was estimated.Methods: Eighty-six inpatients suffering from cerebral infarction in the area of middle cerebral artery (MCA) (32 patients with large area infarction, and 54 with lacunar infarction) and 82 healthy volunteers underwent the TCCS. The hemodynamic parameters of MCA and anterior cerebral artery (ACA), including the peak flow velocity in systolic phase (Vs), mean flow velocity (Vm), flow velocity in end-diastole (Vd), pulsation index (PI), and resistance index(RI) were detected by TCCS separately, and the size, number, location, quality, and hemodynamic parameters of atherosclerosis plaques were measured by trascranial Doppler sonography (TCD).Results:1. There were significance in flow velocity (FV) in patients with large area cerebral infarction in the injured side of MCA, uninjured side of MCA (P<0.01), and control group (P<0.05). There were highly significant difference in RI and PI between patients and control group (P<0.01), but not between injured side and uninjured side (P>0.05).2. FV of injured side and uninjured side of ACA reduced obviously (P<0.05) in the large area cerebral infarction group and control group, while there were no difference between the two sides. There were no significant difference in RI between injured side and uninjured side (P>0.05), but the RI of both sides are much higher than control (P<0.01). Significant difference of PI could not be observed between injured side and uninjured side (P>0.05), while PI of both sides increased notably to compare with control (P<0.01).3. In comparison with healthy volunteers, Vd of each side of MCA in patients with lacunar infarction cut down noticeably, and the injured side was even worse. Not like the uninjured...
