Objectives: This study approach the influence of Trimetazidine (TMZ) on the neuronal apoptosis, the expression of Bcl-2 protein and Caspase-3 protein during the focal cerebral ischemia and reperfusion injury.Methods: This research makes use of the focal cerebral ischemia and reperfusion model induced by a suture method which preferred by Zea Longa in rats. The rats randomly allocated to sham-operation group, ischemia and reperfusion group (3,6,12,24,48h) ,TMZ group (3,6,12,24,48h) . In the ischemia and reperfiision group and TMZ group at prescriptive time, rats were anesthetized and executed. To observe brain histopathological changes after focal cerebral ischemia and reperfusion, HE staining, TUNEL method and immunohistochemistry were used to observe Trimetazidine's influence of the Bcl-2, Caspase-3 expression in the focal cerebral ischemia and reperfusion tissue at different time.Results: 1 .The pathological changes: the hemispheres in ischemia and reperfusion group swelled heavily and seemed pale. HE staining showed the morphology of neurons, neuroglia and capillaries of sham-operation group was normal. In ischemia and reperfusion group, the local neuronal necrosis was observed in the ipsilateral external corpora striatum and cortex of parietal lobe, such as neuron swelling, nuclear fragmentation and lysis. The neurons in the ipsilateral internal corpora striatum and cortex of frontal lobe were very little in size and amount, and the space between them became large, there existed a lot of apoptotic neurons, showed karyopyknosis, chromatin concentration, nuclear fragmentation, et al.2. Apoptotic cell showed brown nuclear in the TUNEL assay. There were few positive cells in the sham-operation group, while in the operation group, appeared in both central and peripheral area.
