It is undoubted that so far organ transplantation and hematopoietic stem cell transplantation (HSCT) have been one of the most effective ways to cure various organ disfunction, organ malignancy and malignant blood disorders. After organ transplantation, whether the graft can survive or not is largely dependent on the matching extent of HLA between the donor and recipient pairs. However, when allogeneic organ transplantation is performed, the biggest barrier to be overcomed is to select a suitable donor whose HLA is fully matched with the recipient. The essence of this barrier is that HLA has the genetic characteristics of the highest complexity, polymorphism and diversity.With the great progress of technological development, PCR (polymerase chain reaction) has been applied in HLA gene typing and the numbers of HLA I and II alleles are growing with a striking speed. Up to Jan 2006, the sum of HLA alleles has been accumulated over 2337, which undoubtedly increases the difficulty of transplant donor selection. Since 1990s the establishment and perfection of Bone Marrow Donor Bank in many countries around the world have greatly relieved the awkward situation of potential transplant recipients lacking their matched donors. But HLA shows apparent racial specificity, so its allele frequency is different in different races or in different groups of the same race. In order to increase the chances of finding the HLA matched donors and to improve the accuracy of HLA typing and at the same time to avoid leaving out the undetected novel HLA alleles, in addition to increase the amount of marrow donor registration, detection and investigation of the HLA alleles in specific people have always been an important subject and technological competing hot spot in current international HLA research field.
