Objective: To observe the effect of Second Trimester Pregnancy Induced Labor with New Compound Castor Meal(NM) and explore the Mechanism of the Labor Launching. Methods: To observe the effect of Second Trimester Pregnancy Induced Labor with NM from two sides of the clinic and animal experiment. Then examine the changes of correlated indexes including prostaglandin(PGs) ,oxytocin(OT), endothelin(ET) and the tissue pathology influence of uterus, placenta and caul. During the clinic study, eighty second Trimester Pregnancy objects were divided into four groups randomly: New Compound Castor Meal cooperating with Mifepristone-Misoprostol(NCMM) group, NM group,Mifepristone-Misoprostol(MM) group and Ethacridine group. There are twenty objects in Each group. Induced Labor effects, intervals from dose to uterus contracting appearance, general delivery time, integrity degree of discharged placenta and caul, remnant tissue quantity in uterus cavity, rate of clearing uterus cavity, bleeding hour, bleeding quantity and side effects of digestive system were observed after taking medicine and the tissue pathology influence of placenta, caul were also observed. During animal experiment, thirty five pregnancy big rates were divided into five groups randomly: NCMM group, NM high dosage group, NM low dosage group, MM group and model group(without dose). There are seven big rates in Each group. Induced Labor effects were observed and PGs (6-Keto-PGF1α) /TXB2),OT, ET were examined. Then the tissue pathology influence of uterus, placenta and caul were also observed.Results:(1)Clinic study indicates that the rate of induced labor success between NCMM group and MM group allreach 100%, which is significantly different from NM group that was only 80% (P<0.05) and appear increasing trend for Ethacridine group that was 95% (P>0.05) .Intervals from dose to uterus contracting appearance and general delivery time were shortened in NCMM group,NM group and MM group, which is significantly different from Ethacridine group(P<0.01, P<0.05) . Observed groups in the integrity degree of discharged placenta all achieved 100%. The integrity degree of discharged caul in NM group and MM group were75%, 25%, which is significantly different (P<0.01) .On the aspect of the remnant tissue quantity in uterus cavity and bleeding quantity, NMgroup and MMgroup is least(P<0.01) .For the rate of clearing uterus cavity, side effects of digestive system, bleeding hour, age, bearing quantity and pregnancy weeks, all groups were not significantly different(P>0.05) .To the tissue pathology observation, the placenta and caul of NCMM group were damnified observiously.(2) Animal experiment study indicates that the rate of induced labor success between NCMM group and MM group all reach 100%, which is significantly different from NMgroup that is 42.85% (P<0.05 ) .Except MM group, the other groups can all increase the level of TXB2 and 6-Keto-PGF1α (P<0.01 or P<0.05) .For the changes of ET,OT, all groups were not significantly different (P>0.05) . To the tissue pathology observation, the placenta and caul of NCMM group were damnified observiously. Conclusions: Dose project of Second Trimester Pregnancy Induced Labor with NCMM are effective in the rate of induced labor success, integrity degree of discharged placenta and caul, remnant tissue quantity in uterus cavity, bleeding quantity ,which also possess the trend of shortening intervals from dose to uterus contracting appearance and general delivery time rate of clearing uterus cavity. The main mechanism of the project was to aggreviate local issue damnification and increase the level of bleed TXB2 and 6-Keto-PGF1α,which is not related to the level of bleed OT, ET.
