OBJECTIVE: To compare the analgesic effect of Lappaconitine injection into Yaoshu point among the different dosage groups and to investigate the most effective dosage administered which gained the best analgesic result.METHODS: 40 patients with combined hemorrhoids were enrolled and divided randomly into four groups: control group (group A); dosage of Lappaconitine 4mg(group B), 8mg(group C), and 12mg(group D) groups. Milligan-Morgan operation was routinely performed in four groups. Pain scores were recorded at 3,6,12,24,48 and 72 hours after operation. Pain scores was assessed by visual analogue scale (VAS) (0 indicated painlessness and 10 points indicated the most severe pain). Side effects of Lappaconitine injection into Yaoshu point (such as dizziness, dyspnea, palpitations, itch, rash, nausea or vomiting), as well as the complications (such as perianal edema, fecal incontinence, urinary retention or constipation) were observed, respectively.RESULTS: Subjective pain response were significantly less in the Lappaconitine group; and there had correlation between the effect and the dose; moreover, the method of Lappaconitine injection into Yaoshu point did not increase the occurrence of postoperative complications and side effects. The incidence of perianal edema was also lower in all the Lappaconitine groups; and the higher dosage we administered, the more significantly the effect of edema regression appears.CONCLUSION: ①Postoperative pain after hemorrhoidectomy can be safely controlled by using Lappaconitine; ②Preoperative dosage of Lappaconitine 4~8mg (2ml:8mg) injection into Yaoshu point is optimal and satisfactory for the Postoperative analgesia; ③ Properative application of Lappaconitine injection into Yaoshu point can prevent the perianal edema after hemorrhoidectomy;...
