Objective To observe the therapeutic effect of femoral head avascular necrosis by core decompression and bone graft through trochanter base on the ARCO classfication. Methods Eighty hips of 65 cases suffered of avascular necrosis of the femoral head were treated with core decompression and bone graft through trochanter. According to the ARCO classification, 80 hips of 65 cases were classified into stage I (23 hips), II (41 hips),â…¢(16 hips); 80 hips of 65 cases were classified into groups A (25 hips),B (25 hips),C (30 hips) according to the area of osteonecrosis. The outcome were evaluated by the function changes of hip joints , progression in clinical and radiographic . Results The patients were followed up 24~72 months, with an average of 40 months.The grade after operation was better than the grade before operation(P<0.05) in stage I(â… A,â… B,â… C )and stageâ…¡(â…¡A,â…¡B,â…¡C). The grade after operation was better than the grade before operation(P<0.05) in groups A,B,C .In stage I, there were 14 hips excellent,5 hips good,4 hips fair,0 hip bad; In stageâ…¡, there were 14 hips excellent,13 hips good,11 hips fair,3 hips bad; In stageâ…¢there were 0 hip excellent,5 hips good,4 hips fair,7 hips bad. The excellent and good rate were 82.61% ,65.85%,31.25% in stage I, II,â…¢respectively, There were significant difference between the outcome of every stage ( P=0.0042) . In group A, there were 13 hips excellent,8 hips good,3 hips fair,1 hip bad; In group B, there were 9 hips excellent,7 hips good,5 hips fair,4 hips bad; In group C, there were 6 hips excellent,8 hips good,11 hips fair,5 hips bad; The good and excellent results in groups A,B,C were 84.00%,64.00 %,46.67 % respectively .There were significant difference between the outcome of every groups...
