Objective: Investigate the effect of atomizating Inhaling Compound Salvia Miltiorrhlza injection to treat the acute cerebral infarction disease, discuss its mechanism and evaluate it' s safety.Methods: In the clinical study, totally 98 inpatients were collected.The patients selected were randsomized into four groups: the general western medicine treatment group(GT for short), the western medicine treatment plus atomizating Inhaling normal saline group(GTNIT for short), the western medicine treatment plus Compound-Salvia Miltiorrhlza injection intravenous drip group(GTIVT for short), the western medicine treatment plus atomizating Inhaling Compound Salvia Miltiorrhiza injection group(GTNIT for short). Continusous 14 days completed the period of treatment. Noting the defect severity of clinical neural function totally before and after the treatment, hemorrheology , TCD results and the ill-effect. Dialysis all the groups the melioration of defect severity of clinical neural function, hemorrheology and TCD changes and evaluated its effect and safety. The experimental study adopted the MCAO ways to make the cerebral ischemia-reperfusionmodel , the adult male SD rats were selected, were randsomized into six groups: sham operation group (N for short), model group (MT for short), Compound Salvia Miltiorrhiza injection inreoperitoneal injection group(IP for short) , compound Salvia Miltiorrhiza injection atomization Inhaling of small dosage groups (SD for short), moderate dosage groups(MD for short) and high dosage groups(HD for short). Continuous seven days completed the treatment course. Investigate and note the neural deficit scores of all the rats at different times 2h, 24h, 72h, 7d after reperfusion and the motality was caculated. The content of superoxide diamutase, malonaldehyde and nitricoxide were measured in the homogenate of the rat' s brain. Observe the change of the cytology morphologic and ultrastructure of the rat' s brain tissue pathologic slices through the optic and electron microscope. Results: In our clinical study, the effective rate of all the four groups are respectively 81. 5%, 74.1%, 66. 7%and 64. 7% for GTIT, GTIVT, GTNIT and GT groups. Statistical differences presented in declined scores of severity of neural fuction defect, hemorrheology and TCD change after treatment in four groups compared with their own controls. Not only for iv and atomization Inhaling to using compound Salvia Miltiorrhiza injection to treat, they all had the effect to meliorate the scores of severity of neural fuction defect, hemorrheology and TCD changes, indicating very significantly difference compared with GT and GTNIT groups. But there are no differences between the GT and GTNIT, GTIVT and GTIT groups. There are no ill effect. In the experimental study, The NDS change and mortality in all the dosage groups of compound Salvia Miltiorrhiza injection atomization Inhaling was improved more obviously compared with sham operation group , model group and Compound Salvia Miltiorrhiza injection inreoperitoneal injection group after treatment, indicating very significantly difference. All the dosage groups of compound Salvia Miltiorrhiza injection atomization Inhaling could improve superoxide diamutase vigor, debase the content of malonaldehyde and nitric oxide in the homogenate of the rat' s brain. Through the optic and electron microscope, we could observe that all the dosage groups of compound Salvia Miltiorrhiza injection atomization Inhaling had the different degrees protective effect in the histology and cytology pathological change after reperfusion , presenting definite dosage—dependant trend.Conclusion: Compound Salvia Miltiorrhiza injection atomization Inhaling treated the acute cerebral infarction had the definite effect, not only it could meliorate the neural function defect, heighten the effect, improve the brain blood flow and hemorrheology conditions. And the treatment had the different degrees protective effect in the histology and cytology pathological change after reperfusion, its mechanism is connected with defending against radical damages, alleviating the nitric oxide toxicity.
