Objective To study the clinical experience of the microsurgical operation in the sellar region tumors via the pterional approach .Methods We retrospectively the 46 patients that include all kinds of sellar region tumours which had been operated through the pterional approach in microsurgy Technique during the 2002-12∽2005-9 in the 1st hospital of the Shanxi medical university . Clinical features ,CT or MRI characteristics of the patients were studied. We discussed the factors that can affected the choice of the operative approach .The operative results and complications were discussed also in microsurgical techniques .Results 45 patients had obtained good results of all cases, the sellar region tumours total resection rate were 80.4%.one patient was died after the surgery. two patients were facial paralysis. Five patients were diabetes insipidus and electrolyte disorder.The follow–up survery for ten months there have no tumor recurrence sign.Conclusion The pterional approach used in removing sellar region tumors has the advantages such as the short working distance, minimal injury to the neural structures and sufficient exposure. It can achieved to removed the giant or large pituitary adenoma which growing toward the suprasellar,parasellar and so on ,the craniopharyngiomas, crista sphenoidalis meningioma and the tuberculum sellae meningioma . It is the key to success in total removal of the tumour to be familiar with the relevant anatomy ,sufficiently utilizing the operatives spaces and microsurgical techniques.
