Objective:To summarize and to explore the clinical features andsurgical strategies for sellar region meningioma.Methods: Clinical data of49cases of sellar region meningioma treatedby surgery in our hospital were analyzed retrospectively. Clinical featuresof suprasellar and parasellar meningiomas were analyzed; differencesbetween unilateral subfrontal approach and pterional approach werecompared and surgical strategies of sellar region meningioma weresummarized.Results: Vision and visual field damage and pituitary compress weremore common in suprasellar meningioma.49cases of sellar regionmeningioma,according to Simpson grade,total removal was made in33cases while subtotal removal in14cases and partial removal in2cases.Total removal rate was67.3%.There was no difference inintraoperative blood loss, operative time and postoperative hospital staybetween two main different types of surgical approach(unilateral frontalapproach and pterional approach)(P>0.05).Conclusions: Suprasellar and parasellar meningiomas have different clinical features; vision and visual field damage and compresssion ofpituitary is more common in the former. On the basis of preserving nervefunction and quality of life, maximum resection is the main direction.Unilateral subfrontal approach and pterional approach both achieve goodexposure of sellar region. No difference in intraoperative blood loss,operative time and postoperative hospital stay are observed.
