Objective: Gastric cancer is one of the most common cancers and the most frequent causes of cancer-related deaths. Micrometastasis is believed to contribute to the recurrence and metastasis postoperatively in gastric cancer patients. Micrometastasis in abdominal cavity and peripheral blood has been proved an independent prognotic factor in gastric cancer. This study is to detect changes of micrometastasis in abdominal cavity and peripheral blood in patients undergo laparoscopic radical gastrectomy, so as to investigate the oncological safety of laparoscopic radical gastrectomy in advanced gastric cancer.Methods: 78 patients with gastric cancer underwent radical gastrectomy from December 2004 to March 2006, laparoscopic radical gastrectomy was performed in 41 patients and open radical gastrstectomy in 37 patients. Total RNA was extracted from peripheral blood and peritoneal lavage of those patients with gastric cancer before and after operation. CK-20 mRNA was used as a probe to detect gastric cancer cells in peripheral blood samples using RT-PCR and CEA mRNA was used as a probe to detect gastric cancer cells in peritoneal lavage samples.Results:1. CK-20 mRNA expression in blood was dectected in 14 out of 41 patients (34.14%) preoperatively. The preoperative positive rate of CK-20 mRNA expression in blood in the patients with II phase and III phase was 13.33 %( 2/15), 46.15 %( 12/26) respectively. A significant difference of preoperative expression of CK-20 mRNA was observed between different stages (P<0.05). The preoperative positive rate of CK-20mRNA expression in blood was 34.43 %( 12/37) in open radical gastrectomy (ORG) group. No significant difference of preoperative CK-20mRNA expression was seen between the ORG group and the LRG group.2. The preoperative positive rate of CEA mRNA expression in peritoneal lavage was 34.14 %( 14/41) in the LRG group.
