Objectives: To determine the predictive value of testing of fetal fibronectin combining with Human Chorionic Gonadotrophin in the cervical secretions.Methods: 92 women with symptoms of preterm labor during outpatient or emergency vists were invited to participate. FFN and HCG measurements from cervicovaginal secretions are performed.All 92 women were followed up for gestation.Results: A positive fFN result and higher hCG level were associated with a significant increase in the incidence of preterm delivery.The sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV) of positive fFN for predicting delivering within 7day,14days or before 37 weeks'gestation were 100%,40.5%, 21.7%, 100%; 100%, 47.8%, 41.7%, 100%; 90.7%, 57.1%, 65%, 87.5% respectively. Using a cut-off of hCG≥60 mIU/ml,higher diagnostic accuracy for predicting preterm birth was obtained with positive results in both tests. The sensitivity,specificity,PPV and NPV for predicting delivering within 7days, 14days or before 37 weeks'gestation were 100%, 72%, 37.1%, 100%; 100%, 88.8%, 62.8%, 100%; 91.7%,80%,94.3%,80%, respectively.ConcIusions: Either a positive fFN test result or a higher hCG is a useful predictors of preterm birth in symptomatic patients. The predictive value of hCG in cervical secretions is more effective than that of fFN. FFN combining with hCG measurements improves the accuracy for predicting preterm labor within 14 days.
