Objective: To examine the effects of laparoscopic cystectomy of endometriomas on the ovarian function of the residual ovary and fertility overcome. Methods: Two groups of infertile patients were retrospectively selected from an in vitro fertilization-embryo transfer(IVF-ET) database in our hospital: 70 patients who underwent laparoscopic ovarian cystectomy for endometriomas were selected in the study group, including 39 patients treated for a unilateral endometrioma and 31 patients treated for bilateral endometriomas ;92 patients with tubal facter infertility were included in the control group. Stimulation parameters, fertilization , better embryo, and clinical pregnancy rates were analyzed in both groups. Results : The mean age of patients and the percentage of male infertility were similar between the two groups . The number of gonadotropin ampoules in IVF -ET cycles in the study group was significantly higher than in the control group (P<0.001),but the blood E(2) level and the numbers of dominant follicles observed on the day of hCG injection and the numbers of oocytes retrieved were significantly decreased in the former (P<0.001). The fertilization , better embryo , and clinical pregnancy rates was respectively 60.1%, 68.6%.31.6% and 66.4%, 73.4%, 34.6 % in the endometrioma group and the control group. The fertilization rate in study group was significantly decreased (P<0.01),but the better embryo and clinical pregnancy rates were similar in both groups (P>0.05) .Conclusion: Laparoscopyic ovarian cystectomy may induce a loss of follicle reserve, but it does not impair fertility outcomes in IVF -ET cycles.
