Expectant Management Of Early-onset Pre-eclampsia: Perinatal Outcome

OBJECTIVE: To evaluate the perinatal outcome of expectant management of early-onset pre-eclampsia.DESIGN: 115 early-onset preeclampsia women in January 1999 to December 2004 were classified into two groups by accepetting expectant managemenor not as management group and control group to compare the perinatal outcome. And a clinical follow-up (18 months to 7 year) for long-term infant outcome was done.RESULTS: There were no differences between management group and control group in women's age, gestational weeks of onset, diagnosed, in-hospital, systolic pressure, diastolic pressure, urine protein in 24h. But fetal and perinatal deaths rate in management group is much lower than control group (3.09% vs 26.3%; 7.22% vs 34.2%).A mean of 10.7 days were gained by expectant management. Significant differences in favor of expectant management were noted in birth weight(1806g vs 1515g), Apgar score in 1 and 5 minute(8.75 vs 7.19; 9.54 vs 8.81), time for NICU(12.3d vs 19.7d). Complications in the two groups are similar. A clinical follow-up (18 months to 7 year) was done for long-term infant outcome, including height, weight, fedding, speech, physical and mental development, defect in listening vision and physical, the time and reason of death. 51 fellow-up were gained, 37 in management group and 14 in control group. There was one death in management group, and the reason is unknown. No death in control group. Visual defects exist in four cases. The incidence of complications such as physical defect and mental defect are very low.CONCLUSION: It is safe to give expectant management to early-onset severe pre-eclampsia, and the perinatal outcome is well.
