Correlation Study Of Uterine Arteriography Embolization And Ovarian Function

ObjectiveTo investigate arteriography analomy and study the correlation study of super selective uterine artery embolization and ovarian function.Material and method1. Patients'material60 patients of uterine leiomyoma were treated by uterine artery embolization(UAE)(age range 41-58 years old, mean age 48 years old). All patients were diagnosed by B ultrasound. 10 patients had leiomyoma laying were under mucous membrance. 18 patients were in the myometrium. 25 patients were under mucous membrance and in the myometrium. 7 patients were under serious membrance and in the myometrium. 31 patients had multiple leiomyomas. 29 patients had single leiomyoma.2. Arteriography and uterine artery embolization measure Operation should be performed during 3-10 days after menstrual. Underanesthesia, seldinger's technology centesis was adopted in illiac arteries of one side. 4F cobra tubes were super-selectively inserted into uteries of two sides. Under vascular image, PVA was injected into the arteries of the myoma. The total amount of PVA grain for every patient was 100-300mg.3. Observation of follow-up and testing of ovarian hormones level Follow-up observation should be done during 6-60 months afteroperation. The volum of uterus ,myoma, ovary should be measured.History of clinical symptoms and pelvic signs were taken. Changes of mense were observated. The sexual serum level was observed before and after operation.4. According to Spies, the calculation formula is A×B×C×0.523. The multiple myoma is measured by the biggest one.5. StatisticsTo use SPSS 10.0 statistic software, mean analysis,T-test,ANOVA and SNK. The size of test was determined by α=0.05.Results1. The origination of the uterine arteries and its branchesIn 120 internal iliac arteries,72 uterine arteries originate from the anterior trunk of interal iliac artery. 32 uterine arteries originate from the trunk of inferior gluteal and internal pudendal artery. 10 uterine arteries originate from the internal pudendal artery. 2 uterine arteries originate from superior gluteal artery. 70 uterine arteries originate show the angle of origination by < 90.32 show =90.18 show > 90°. Three projeted positions (posteroanterior, ipsilateral oblique and contralateral oblique view) were taken and the view aspects of internal iliac arteries were observed. Contralateral lique view is the best projected position to show the opening and route of the uterine artery.2. Most of the cases with uterine fibroids show bilateral dominant artery feeding. The volumes of the myoma of the cases in the 3^rd,6^th,12^th month after UAE were analysed with significant difference. The mean leiomyomas volune reduction at 3 months,6 months, 12months. After UAE were 40%,55%,and 60% respectively.3. Serum levels of sex hormones had no significant difference before and after UAE. Most patients resumpte normal mense. Nobody was in menopausal.ConclusionSuper-selective rterine arterial embolization with PVA is a confirmed method for treatment of uterine myoma. UAE has no manfast effect on endocrine function of ovary. It has certain effect, micro injury. And it can reserve uterus. So it is a safe, effective method.UAE expand the field of the treatment of uterine leiomyoma and interventional radiology.
