Abstract/Summary:
Premature rupture of the membranes (PROM) is rupture of the membranes before being about to give birth. Rate of PROM is about 10-20% of all delivery. PROM is a complication affecting of labor. PROM is the main known cause of preterm delivery. PROM can result in infection of neonatal and fetus predicament in uterine, and is associated worldwide with increased rates of neonatal and maternal morbidity and mortality during prenatal period. With the progress of the society, people's awareness towards eugenics has universally ascended to a new level, and they generally attach great importance to the prenatal mortality and the factors that affect fetal development. Unfortunately, the etiology and pathogenesis of PROM remain an enigma. It brings lots of troubles for prevention of PROM.In the course of pathogenesis of PROM, cellar apoptosis of the membranes tissues and relevant genetic regulation are gaining increasing attention from lots of researchers. The cellar apoptosis and necrosis are two different concepts. Apoptosis is a gene- regulated- programmed cell death. It acts as a basic physical event, taking place in all kinds of physical and pathologic conditions. In the research of membranes apoptosis gene, researchers have confined that apoptosis is present in membranes from the pregnant women with PROM. This gene controls cellar apoptosis, therefore it leads to the pathologic changes of membranes. Nevertheless the pathogenesis of PROM is far more than complicated. It is probable a multi-gene-controlled disease, which is also subject to environmental factors. At present, the prospective genes that are relevant to PROM have been identified, and the total number is 10. However, which gene is present in the membranes tissues, how about their genes expression, how the genes contribute to the apoptosis and what mechanism of apoptosis? For above questions, we lack systematic research both at home and abroad.This experimentation intends to observe the apoptosis of the pregnant women with PROM using light microscope and transmission electron respectively. We explore the degree of gene expression of bax and bcl-2 and how they facilitate the apoptosis to explain the pathogenesis of PROM, for the sake of gaining some theoretical support for the predictive prevention and treatment of PROM.Materials and methodsI collected the membranes tissues of 44 pregnant women with PROM and 26 normal full term pregnant women with no PROM and made a comparison between the two groups of the gestational age and times of delivery. These are no statistical significance. First I made sure if the apoptosis is present in the above samples using light and transmission electron microscope, then I adopted techniques of immunochemistry and histochemistry to explore the gene expression of bcl-2 and bax.Results1. The apoptosis is present in the trophoblastic cells, epithelial cells of amnion and the stromal cells of pregnant women with PROM and normal pregnant women, especially the trophoblastic cells (cells of cell's trophoblast and the cells of syncytical trophoblast).2. The experimentation proves that the positive ratio and expression levels of bcl-2 protein is statistical significance(P=0.0025<0.05)in both the membranes tissues of the pregnant women with PROM and the comparative group. The positive ratio and expression levels of bax protein is statistical significance(P=0.0086<0.05)in both the membranes tissues of the pregnant women with PROM and the comparative group.3. Among the membranes tissues of 44 with PROM, bax and bcl-2 don't have a correlation each other [rs=0.07162(P=0.5558>0.05)].Conclusion 1. The pregnant women of term with no PROM have some of apoptosis in the membranes tissues.2. Apoptosis in the membranes tissues of PROM is more obvious than that of no PROM. Apoptosis of outstanding difference may be main route of pathology bringing about PROM.3. Expression unbalance of both bax and bcl-2 is action mechanism which apoptosis genes in the membranes result in apoptosis.
