Objective To investigate the ultrasturctural changes of small intestinal mucosa in cirrhotics and discuss it's significance.Methods Small intestinal mucosas of 60 cirrhotics and 18 volunteers were obtained by routin endoscopic biopsy. The ultrasturcture of small intestinal mucosas of these subjects were observed with transmission electron microscopy. The cirrhotics were divided into group A, B and C according to Child-pugh classfication criterion, and the volunteers were served as controlled group The rates of ultrasturctural change of small intestinal mucosa of each group were compared. Cirrhotic patients with and without ultrasturctural changes of small intestinal mucosa were divided into positive group and negative group. The levels of the ALB, TBIL, PT, plasma endotoxin, blood ammonia of positive, negative and controlled group were measured and compared.Results There were 20 patients in each group of group A, B, C. Ultrasturctural changes of small intestinal mucosa were found with transmission electron microscopy in 5 patients of group A, 9 patients of group B, 17 patients of group C, totally 31 patients of 60 cirrhotics, the rate of change is 52%. These changes include the rare, ruptured, arrange indiscriminate microvilli on the cells of small intestinal mucosa, karyopyknosis and karyorrhexis, widen gap of tight junction and the tumefaction of mitochondrion of the cells of small intestinal mucosa. No ultrasturctural changes of small intestinal mucosa were observed in controlled group.The rates of change in each Child-pugh class group and controlled group were 25 % , 45 % , 85 % , 0 % , P<0.05.The ALB in the positive group was significant lower than that of negative group, P<0.01.
