Objective:To evaluate the left ventricular myocardial regional systolic and diastolic function of Epirubicin-induced cardiotoxicity using strain rate imaging(SRI).To investige the Eprirubincin-induced cardiotoxity in breast cancer patients by integrated backscatter( IBS) . To investige the left ventricular cardiac function in breast cancer patients treated with Epirubicin induced myocardial damage, and quantitative tissue velocity imaging (QTVI) was used to detect Tei index.Methods:133 breast cancer patients were divided into three groups:fourty-five patients in group A were given Epirubicin(Epi)of 180~240 mg/m2,fourty-eight patients in group B were given Epirubicin of≥360 mg/m2,and fourty patients after surging without chemotherapy in group C as the control group. The parameters of the conventional echcardiographic, SRI,IBS and QTVI were respectively measured.And the conventional echcardiographic parameters were also obtainted.Obtained Parameters inclued 1. The conventional echcardiographic parameters left ventricular and diastolic diameter(LVDd),left ventricular and systolic diameter(LVDs), interventri-cular septun thickness diastolic(IVSTd),left ventricular pesterior wall thickness diastolic(PWTd)), left ventricular ejection fraction(LVEF),fr- action shortening(FS), E velocity /A velocity(E/A).2. SRI parameters The parameters of systolic strain rate(SRs),early diastolic strain rate(SRe)and later diastolic strain rate(SRa)were respectively measured. 3.IBS parameters The integrated backscatter (IBS),cyclic variation of integrated backscatter( CVIB) were obtainedand examined IBS%, TGCV(transmural gradient of CVIB),TG(Itransmural gradient indexof CVIB).4.Tei index the isovolumic relaxation time (IRT),isovolumic contraction time (ICT) and ejection time (ET) of LV were obtained using QTVI,and examed Tei index.Results:1.The conventional echcardiographic parameters had no significantly difference between groups A and C,groups B and C(E/A was not included)(P﹥0.05),but E/A was considerably lower in group B,had significant difference between groups B and C(P<0.01).2.SR parameters SRI parameters had no significantly difference between groups A and C .The SRs is lower in group B,but it had no significant difference between groups B and C, groups B and A. SRe and SRa which were considerably lower in group B,had significant difference between groups B and C, groups B and A(P<0.01).3.IBS parameters The parameters of CVIB, IBS%, TGCV, TGI were no significantce difference between groups A and C(P﹥0.05). The CVIB,TGCV,TGI were considerably lower, the IBS% was considerably higher in group B. They were significant difference between groups B and A, groups B and C(P<0.01 or P<0.05).4. Tei index The Tei indices had no significant difference between groups A and C(P﹥0.05)but there were significant difference between groups B and C, groups A and B(P<0.01).Conclusions:1. SRI may be valuable to assess the left ventricular myocardial regional systolic and diastolic function in patients treated with Eprirubincin-induced cardiotoxity.2. IBS may be valuable to assess the Eprirubincin-induced cardiotoxity in breast cancer patients.3. Tei indexes maybe valuable to assess the left ventricular function in breast cancer patients treated with Epirubicin-induced cardiotoxity.
