Objective To compare the effect and cost of laparoscopic resection for rectal carcinoma with those of the open laparotomy procedure.Methods 15 laparoscopic and 36 open radical operation cases selected from the same surgical candidate ,according of the different types(dixon or miles),were divided into two groups and compared under the following parameters: surgical guidelines, tumor radical resection guidelines ,postoperative recovery guidelines, operation fee,postoperative fee and hospitalization fee.Results In the dixon group,the laparoscopic group(dixon), mean operation time (3.66±1.02h),mean operative blood loss (185.71±159.99ml), tumor size (3.43±1.72cm), number of lymph nodes excised (9.43±1.40), and the distal margin (3.54±1.65cm) ,bowel function restoration time (2.29±0.95d), urinary catheter removal time (2.71 ± 1.60d), operation fee (13542.47 ± 5153.16 ¥),postoperative fee (5044.04 ±1055.08¥),the total fee (18586.51 ±5032.55 ¥ ).When compared to the open group(dixon), no statistically significant differences were founded in tumor radical resection guidelines(p>0.05),the operation time of lparoscopic group(dixon) was longer than the open group(p<0.01). Laparoscopic operation fee and hospitalization fee were more expensive than open group(p<0.01).While in the miles group, the laparoscopic group(miles), mean operation time (3.61±1.45h),mean operative blood loss (468.75±315.03ml), tumor size (3.69±1.71cm), number of lymph nodes excised(8.63±2.00), and the distance from anus (3.15±1.44cm) ,bowel function restoration time (2.25±0.89d), urinary catheter removal time (4.88 ± 2.70d), operation fee (6677.32±1282.09¥),postoperative fee (5102.77 ±1327.00¥),the total fee (11780.09±2128.67¥) . When compared to the open group(miles), no statistically significant differences were founded in surgical guidelines, tumor radical resection guidelines ,postoperative recovery guidelines and hospitalization fee.Conclusions Laparoscopic radical operation on rectal cancer is a technique that is safe, feasible, and less invasive compared to open surgical resection. And when laparoscopic miles surgery compared with open miles in the hospitalization, there is no statistically significant differences. Laparoscopic miles surgery is easier and much more economical than laparoscopic dixon surgery,may be more easier for the generalization in furture.
