Clinic Study On The Effects Of Qindan Capsule In Treating Essential Hypertension Patients Of "Liver Fire And Blood Stagnation"

Background and objective: To investigate the clinical curative effect of "Qindan Capsule" in treating essential hypertension patients of "Liver fire and blood stagnation",and the effect on patients' system of fibrolysis and blood clotting transforming growth factor- β₁ (TGF- β₁),vasoactive substance,the variance of day-night rhythm of blood pressure(BP);to approach Qindan capsule's possible mechanism of action.Methods: 61 subjects were divided into treatment group(31 subjects) and control group(30 subjects). Treatment group were treated by Qindan capsule and enalapril;and concrol group were treated by enalapril for 8 weeks.The clinical symptoms,blood pressure, plasma fibrin D-dimer level, plasma GMP-140 level ,plasma ATIII activity and plasma TGF- β₁ level were detected before treatment and after treatment.20 subjects were choosen from treatment group to make up new treatment group,and 20 subjects were choosen from control group to make up new control group. The plasma endothelin(ET) level,plasma calcitonin gene-related peptide (CGRP) level,plasma angiotonin II (Ang II) level,the variance of BP and degression rate of night BP of patients in new groups were detected before treatment and after treatment.Results: (1)After treatment,in treatment group , the excellence rate of curative effect on clinical symptoms was 41.94%,and it's total effective rate was 93.55%,which was superior to those of control group(P<0.01);(2) the excellence rate of blood pressure degression was 58.06%,and it's total effective rate was 93.55%, which was superior to those of control group(P<0.05);(3) the plasma fibrin D-dimer level was degraded, the plasma ATIII activity was improved(P<0.01),the plasma GMP-140 level was degraded(P<0.05),and there were significant differences comparing with control group in the change of all the three makers (P<0.05);(4)the plasma TGF- β₁ level was degraded (P<0.01),which was superior to that of control group (P<0.05);(5)the plasma ET level and plasma AngII level were degraded and plasma CGRP level was improved(P<0.01), and there were significant differences comparing with control group in the change of plasma ET level and plasma CGRP level (P<0.05),but there was not significant difference comparing with control group in the change of plasma Ang II level(P>0.05);(6) the monitoring result of ambulatory blood pressure showed:the standard deviation of the variance of systolic blood pressure(SBP) and diastolic blood pressure(DBP) in both day and night were diminished(P<0.01); the standard deviation of the variance of SBP and DBP in day were superior to those of control group(P<0.05, P<0.01); there were not significant differences comparing with control group in the standard deviation of the variance of SBP and DBP in night(P>0.05); degression rate of both SBP and DBP in night were increased(P<0.01); there was not significant difference comparing with control group in the change of degression rate of SBP in night (P>0.05);the change in degression rate of DBP in night was superior to that of control group(P<0.05).Conclusion:Qindan capsule can treat the essential hypertension patients of "Liver fire and blood stagnation". After treatment, the BP of patients were degraded, both the clinical symptoms and prothrombotic state were improved, the plasma TGF- β₁ level was degraded, the standard deviation of the variance of BP was diminished and the degression rate of night BP was increased;the mechanism in depressing BP of Qindan capsule was relate to its effect on patients' vasoactive substance and the action of blood vessel endothelium.
