OBJECTIVES: To investigate the relationship between insulin receptor gene of exon-8 Nsi-I polymorphism and cerebral infarction with type 2 diabetes mellitus ; To debate the mechanism of the difference between DMCI and DMNCI. To research the etiology of the DMCI.METHODS: (1) 186 cases of CI were assigned into DMCI (47cases) and DMNCI(139cases). Take the 114 cases as normal control group.(2) case-control designs were used , genomic DNA was extracted from peripheral blood leukocyte of all the cases.(3) Polymerase chain reaction (PCR) was used to detect the polymorphism.(4) The results were expressed as (x|-)±s, The SPSS 10. 1 statistic software was used in analyzing the results by ANOVA,x~2 test.RESULTS: (1) there was significant difference in BS,TG, HDL,VLDL,apoA,apoB,insulin,ISI between DMCI and DMNCI.(2) The damagement signs of neural function after DMCI was much more serious than DMNCI.(3)There was significant difference in N2 allete frequency of INSR-8exon in follow groups:①DMCI and control(P=0. 0007). ②DMCI and DMNCI(P=0. 0081).③CI with hypertension and CI without hypertension(P=0. 0001).CONCLUSIONS: (1) The age of cerebral infarction with diabetes mellitus is younger than cerebral infarction without diabetes mellitus. The damagement sign of neural function after DMCI is much more serious than DMNCI.The BS,high blood pressure and fat metabolism imbalance maybe in active.(2)Nsi-I polymorphism of INSR gene was associated with the Cerebral infarction with diabetes mellitus.(3) Polymorphism of INSR gene affects rehabilitation of the DMCI by the gene and protein expression.
