Objective: To investigate biological behavior of gastrointestinal epithelia metaplasia in the process of ovarian mucinous tumors by detecting the expression of MUC2,MUC5AC,CDX2,NF-κB,p53,p16,K-ras, and make it known that the gastrointestinal epithelia metaplasia is a dangerous phase phenomenon of ovarian mucinous tumors.Methods: 81 paraffin specimen of primary ovarian mucinous tumors were collected. IHC were used to detect the protein of MUC2,MUC5AC,CDX2,NF-κB,p53,p16,K-ras, saturated hydroxybenzene chloroform were applied to gain the DNA from 30 paraffin tissue and PCR-SSCP were to examine the delation and mutation of p53,p16,K-ras in primary ovarian mucinous tumor.Results: (1)There were different expression rates of MUC2, MUC5AC, CDX2, NF-κB in ovarian mucinous tumor. The positive expression rate of MUC2 in benign, borderline and malignant ovarian mucinous tumor were 32.35%,85.19% and 70%; MUC5AC were 100%,96.30% and 50%; CDX2 were 11.76%,33.3%,55%; NF-KB were 0,25.93% and 60%. There were different significance of MUC2 between benign and boderline, unmetaplasia and gastic type in benign group, gastric and intestinal type in boderline(p<0.01) while MUC5AC between benign and boderline, boderline and malignant, gastric and intestinal type in borderline(p<0.05). There were also different significance of CDX2 between benign and boderline, gastric and intestinal type in benign (p<0.05) while NF-κB between benign and boderline, boderline and malignant, gastric and intestinal type in malignant(p<0.05).(2)There were different expression rates of p53,p16,K-ras in ovarian mucinous tumor. The positive expression rate of p53 in benign,boderline and malignant were 0, 11.1% and 35%, p16 were 8.8%, 33.3% and 35%, K-ras were 5.9%, 40.7% and 65%. There were different significance of p53 between benign and boderline, gastric and intestinal type in boderline(p<0.05) while p16 between benign and boderline(p<0.05), K-ras between benign and boderline, boderline and malignant, gastric and intestinal type in benign (p<0.05).(3) There were correlations among MUC2, MUC5AC, CDX2, NF-κB, p53, p16 and K-ras genes. There was positive correlation between MUC2 and CDX2(r=0.353, p<0.01), MUC2 and NF-κB (r=0.232, p<0.05), CDX2 and NF-κB(r=0.460, p<0.01), p53 and p16(r=0.234, p<0.05), p53 and K-ras(r=0.454, p<0.01), p16 and K-ras(r=0.474,p<0.01). There were also negative correlation between MUC2 and MUC5AC(r=-0.230, p<0.05), MUC5AC and CDX2(r=-0.369, p<0.01),MUC5AC and NF-kB(r=-0.446, p<0.01).(4)There were p53, p16 and K-ras mutations in ovarian mucinous tumor.The mutation rate of p53, p16 and K-ras was 20%,13.3% and 20% separately.Conclusions: (1) MUC2 , MUC5AC were positive in the process of tumor generation . It was proved that the cervical type epthelia can metaplasia to gastric type and gastric type can be cancerated or it can metaplasia to intestinal type and then cancerated. (2)As to the ovarian mucinous tumor, gastric type is more than the intestinal type in the benign group, equal in the boederline group and less in the malignant. Genes are further derangement in the intestinal type epthelia. (3) There were significance expression of NF-κB,p53,p16 and K-ras in ovarian mucinous tumor. It suggested that there were genes abnormality in the process of epithelia metaplasia and cell proliferation.
