Objective: To study metabolism of glucose and lipids in first-episode drug-naive patients with schizophrenia and their first-degree relatives and identify if blood glucose and lipids are endohenotypes of schizophrenia.To explore the association between rs16147 polymorphism in the neuropeptide Y gene and schizophrenia. To search for the relation between rs16147 polymorphism(NPY) and blood glucose and lipids.Methods: (1) In this cross-sectional study, oral glucose tolerance , insulin, C-peptide, lipids, body height, body mass, waist circumference and hip circumference were measured in 65 first-episode drug-naive patients with schizophrenia , 22 their first-degree relatives and 24 healthy comparison subjects.The patients ,first-degree relatives and comparison subjects were matched in items of age, gender, and body mass index(BMI). The concentrations of serum insulin and C-peptide were measured by the radiant immuning methods. (2) The rs16147 polymorphism(NPY) was analyzed by PCR-RFLP to investigated the occurrence of the polymorphism in 150 schizophrenia patients,99 first-degree relatives and 113 comparison subjects.The iterms about metabolism of glucose and lipids in three genotypes of patients were compared.Results: (1)The morbilities of IFG, IGR, diabetes and impaiered glucose metabolism are 12.31 %,32.8%,8.6%,43.1 % respectively in patients, which are significantly higher than first-degree relatives and controls. (2) Compared with first-degree relatives and healthy subjects, the patients with schizophrenia had significantly higher OGTT 2h plasma glucose, fasting plasma insulin, OGTT 2h plasma insulin, insulin resistance,fasting C-peptide, OGTT 2h C-peptide. There was no any difference between first-degree relatives and controls. (3) No significant difference was noted in the items of waist circumference, WHR, TG, TC,HDL and LDL among the patients with schizophrenia, first-degree relatives and controls. (4) No significant differences were found between genotype or allele frequencies of rs16147 polymorphism(NPY) in the patient ,first-degree and control groups.The iterms about metabolism of glucose and lipids of three genotypes had no significant difference.Conclusions: (1) First-episode, drug-naive patients with schizophrenia have impaired glucose tolerance, higher levels of OGTT 2h plasma glucose, fasting insulin, C-peptide,eitherway their OGTT 2h plasma insulin and C-peptide, insulin resistant are so as the forementioned. (2) Compared with controls, first-episode, drug-naive patients with schizophrenia have no significant abnormal items about obesity or intra-abdominal fat and lipids. First-degree relatives have no abnormal items about blood glucose and lipids.This result don't support that the items about blood glucose and lipids are endohenotypes of schizophrenia. (3) The rs16147 polymorphisms at the NPY gene are unlikely to be linked with genetic susceptibility to schizophrenia or metabolism of glucose and lipids.
