Objective:Access the significance of detection P16 and HPV16 expression inASCUS to identify high grade squamous intraepithelial neoplasia.Methods:Select the sample 120 patients with atypical squamous cells ofundetermined significance after Thinprep cervicovaginal cytologyexamination and they were done cervical biopsy under the colposcope, cellblock be prepared from residual material of Thinprep Preserve solution ,cellblock section were immunostained with antibody P16 protein and antibodyHPV16.Results:(1)The result of being done cervical biopsy under the colposcope wereas follow: cervicitis in 64 specimens, CINⅠin 33 specimens, CINⅡin 17specimens, CINⅢin 6 specimens, in these cases there were 19.17%highgrade squamous intraepithelial neoplasia (CINⅡ, CINⅢ).(2)Positive rate of immunostaining for P16 on cervical deciduous cellsin cervicitis, CINⅠ,CINⅡ,CINⅢwere 6.25%,51.52%,82.35%,83.33%, respectively. Immunostaining for P16 between cervicitis andCINⅠ, cervicitis and CINⅡ, cervicitis and CINⅢexhibited statistically significant associations(P<0.01), but there were no statistical significantassociation between CINⅠand CINⅡ, CINⅠand CINⅢ(P>0.05). Thesensitivity,the specificity,positive predictive value and negative predictivevalue for immunostaining of cervical deciduous cells for P16 to detect highgrade squamous intraepithelial neoplasia (CINⅡ,CINⅢ) was respectively82.61%,78.35%,47.5%,95%. If assessment aaccording to the standard ,ofintense expression, The sensitivity,the specificity,positive predictive valueand negative predictive value for immunostaining of cervical deciduous cellsfor P16 to detect high grade squamous intraepithelial neoplasia (CINⅡ,CINⅢ) was respectively 78.26%,92.78%,72%,94.74%.(3)Positive rate of immunostaining of HPV16 on cervical deciduouscells in cervicitis, CINⅠ,CINⅡ,CINⅢwere 21.88%,39.39%,76.47%,83.33%, respectively. Immunostaining for HPV16 between cervicitis andCINⅡ, cervicitis and CINⅢ, CINⅠand high grade squamous intraepithelialneoplasia exhibited statistically significant associations (P<0.01), but thereare no statistical significant associations between cervicitis and CINⅠ, CINⅡand CINⅢ(p>0.05). The sensitivity, the specificity, positive predictive valueand negative predictive value for immunostaining of cervical deciduous cellsfor HPV16 to detect high grade squamous intraepithelial neoplasia (CINⅡ,CINⅢ) was respectively 78.26%,72.16%,40%,93.33%.(4) There are significant positive relation between the expression of P16 and HPV16Conclusions:(1)Detecting P16 expression can identify cervical squamousintraepithelial neoplasia among the patients with atypical squamous cells ofundetermined significance. If according to the standard of intense expression,it can raise the accurate nature of detecting high grade squamousintraepithelial neoplasia(2) Detecting HPV16 expression can identify high grade squamousintraepithelial neoplasia among the patients with atypical squamous cells ofundetermined significance.(3)Combined with detecting P16 and HPV16 expression can raise theaccuracy rate of predict high grade squamous intraepithelial neoplasia amongthe patients with atypical squamous cells of undetermined significancerate...
