The Research Of 64-slice Helical CT In The Early Diagnosis Of Solitary Pulmonary Nodules

Objectives To explore the value of the 64-slice helical CT in theearly diagnosis of solitary pulmonary nodules and to ascertain thevaluable evaluation index.Methods The CT is Simens 64-slice helical CT (sensation64).Firstly, the scanning with slice thickness 7mm and slice interval 7mmwas underwent from apex of lung to base of lung. Secondly, the imageswere changed slice thickness 1mm and slice interval 1mm. Thirdly, thepulmonary nodules were were picked out and counted. Lastly , thepulmonary nodules received three-dimensional reconstruction andassessment by "CT soft" and "CT Lung" software.Results (1) Of 23 cases, 11cases is benign (tuberculosis 7 cases,inflammatory pseudotumor 2 cases , hyperplasia 2 cases), 12 cases ismalignant (adenocarcinoma 9 cases, squamae cancer 3 cases). Themalignant cases included I A(T₁N₀M₀) 3 cases,â… B(T₂N₀M₀) 8 cases andâ…¢A(T₃N₁M₀) 1 case.(2) The nodules with diameter greater than 2mm canbe discovered, however, the density and stanard error of nodules＜4mm diameter can not be showed .(3) The percentage of benign diseaseand malignant disease is respectively 56% and 44% in aged≤45-year-oldgroup; the percentage of benign disease and malignant disease isrespectively 44% and 56% in aged 46-59 years group ; the percentage of benign disease and malignant disease is respectively 25% and 75% inaged≥60-year-old group. (4) Micro-small vessels' ingrowth can be foundin all malignant nodules and only one benign nodule. Their differencewas statistically significant(P＜0.01).(5) The difference of malignant ratebetween diameter less than10 nun nodules and nodules greater than10mmdiameter was statistically significant (P=0.022). (6)The coincidence ofimageological diagnosis and pathologic diagnosis is 86.9%. Thesensitivity, specificity and misdiagnosis rate of imageological diagnosis isrespectively 91.7%,81.8% and 18.2%.Conclusions (1) 64-slice helical CT has considerable reference valuefor early qualitative diagnosis of solidoary pulmonary nodules;(2) Thelittle pulmonary nodules＜2mm diameter can be found by 64-slice helicalCT. It can help clinical doctor to discover multiple pulmonary nodulesand to decide whether patient can receive operation.(3) The growth ofmicro-small vessels in nodule is an important referenced index.
