Objective To evaluate endometrial receptivity systematically using transvaginal ultrasonography including grey scale ultrasonography and power Doppler ultrasonography. Methods There were 57 cases received controlled ovarian stimulation cycle, they all were examed about their base conditions before that. It was sure that their both uterine tubes or one uterine tube at least was smooth and their uterine were normal and their ovary can ovulate. At the same time the semen from their own husbands was proved normal. We used the same treatment plan including CC/HMG/HCG,et al. The endometrial thickness were measured and the blood flow distribution patterns in endometrium and subendometrium and the types of endometrial wavelike movements were determined on the day of HCG administration. The blood flow distribution patterns in endometrium and subendometrium were devided into three types, that were the type of I-III, and the types of endometrial wavelike movements were devided into five types, that were the type of (1)-(5) .The frequencies of endometrial wavelike movements were calculated. All cases were devided into two groups according to their pregnancy outcome. The data were analyzed. Results The follow-up information was available for 57 cases(157 menstrual cycles), there are 28 cycles were pregnant, the rate of pregnancy is 17.83%. The mean endometrial thickness in the group of pregnancy was 9.4±0. 8mm ,and it was 9.2±0. 9mm in the other group .The endometrial thickness between the two groups had no significant difference (P>0.05). To the blood flow distribution patterns in endometrium and subendometrium, the rate of type I was 7.14%, the rate of type II was 53.57% and the rate of type III was 39.29% in the group of pregnancy, and the rate of type I was 19.38%, the rate of type II was 65.12% and the rate of type III was 15.50% in the group of non-pregnancy. The blood flow distribution patterns in endometrium and subendometrium had statistical difference (P<0.05) . The endometrial movements were observed in all the 157 menstrual cycles, the types of endometrial movements include positive, negative, opposing and random endometrial movements. Endometrial movements, especially negative movements more frequently appeared in non-conception group than in conception group (P<0.05) .Conclusions Observing the endometrial thickness and the blood flow distribution patterns in endometrium and subendometrium and the endometrial wavelike movements on the day of HCG administration using transvaginal ultrasonography may help to evaluate endometrial receptivity and to predict the pregnancy outcome.
