| Objective:The ability of endometrium to accept embryo is endometrial receptivity(ER),which is one of the key factors for successful implantation of embryo in endometrium. Therefore, endometrial receptivity has attracted more and more attention in the field of reproductive medicine. The research on endometrial receptivity involves basic medicine, clinical cell, molecular biology and other fields in the industry. The basic research on ER molecular biology has been carried out all the time. The research on the positive influence factors of Er formation from gene and gene expression level is concerned by many researchers. However,it is difficult to carry out large sample research due to its need for invasive detection, long test time and high equipment requirements.Due to the small number of research samples and insufficient data, the research results have certain limitations, and more experimental data are needed to confirm and apply the research results to clinical practice. In recent years, due to the rapid development of ultrasound instrument technology, the research on ER evaluation using this method has been reported.Because of its strong operability,non-invasive,simple and economic,it is favored by many people. However, the evaluation of the parameters with higher predictive value or which combination of the parameters is more meaningful, reproductive field and related medical imaging field have different evaluation.According to ER related research results,in recent years, there are many studies on increasing endometrial blood flow to improve endometrial receptivity, suggesting that endometrial receptivity is related to endometrial neovascularization.Endometrial biopsy immunohistochemistry also confirmed the theory. In this study,we used transvaginal B-ultrasound to monitor endometrial thickness, subendometrial blood flow pulsatility index and resistance index The purpose of this study is to compare the endometrial blood flow between the two groups and evaluate whether the endometrial blood flow can be better predicted according to the clinical parameters. This study is to analyze the endometrial and subendometrial blood flow under ultrasound, evaluate endometrial receptivity(measure endometrial thickness, peristaltic wave times(times / min),and subendometrial blood flow parameters, and evaluate its predictive value for ER.Methods:to analyze the outpatient data of reproductive center of Ma’anshan maternal and child health hospital from June 2019 to June 2020.Follow up the pregnancy or not, compare the endometrial thickness and blood flow parameters of pregnant and non pregnant patients, analyze the clinical value of these parameters in evaluating endometrial receptivity and predicting pregnancy rate. Results: among the 200 cases,60 cases reached the standard of clinical pregnancy, and the clinical pregnancy rate was 30%.The thickness of endometrium, peristalsis and blood flow parameters were measured on the 6-8day after ovulation or on the day of transplantation: pulsatility index(PI),resistance index(RI),systolic peak blood flow velocity and diastolic blood flow velocity ratio(S/D).Conclusion:The endometrial thickness of pregnancy group was higher than that of non pregnancy group.RI in pregnancy group was lower than that in non pregnancy group. |
