| Background For the past few years, the problem of bacterial drug resistancehas been pay close attention in worldwide.nosocomial infections caused by pers-ister not only prolong the hospitalization time of patients but also increase themedical cost, and threaten to the lives of patients.Acinetobacter is an opportune-istic pathogen, following the increase of Acinetobacter's infect rate, there is m-ore and more attention pay to it. According to the monitoring data of nosoco-mial infections by American and pathoge-nic bacteric investigation of nosocom-ial infections by China. Acinetobacter infection is the fourth in nosocomial in-fections and it is another non-fennentable suagr bacter second only to Pseudo-monas aeruginosa, and even exceed to Pseudomonas aeruginos-e in some place.Along with the widespread use of broad-spectrum antibacterials, mul-tidrug resist-ant Acinetobacter is increased graduaUy.The persister is resistant to all the antibacterial agents, and can easily cause epidemic outbreak in hospital through cro-ss-transmission, it takes a great difficult to the clinical anti-infective therapy.Butin t-he genospecies of the Acinetobacter, Acinetobacter baumannii is the highestin clinica-1 isolating rate, drug-resistant rate and intractable in medication.The stu-dy shows that the drug-resistant information of Acinetobacter has regional disp-arity, different country or region, even in different division of the same hospital, it has different drug-resistant information.The investigation of Acinetobacter inf-ections in general hospital has kee-p on a long time, and obtained much achiev-ement.But now, there aren't any reports of Acinetobacter infections in Childrenspecial hospital.For understand the infection and drug-resistance of Acinetobacte-r in children which is a low-immunity group, we car-ried out the following stu-dy.Objective To study the epidemiology of Acinetobacter and Acinetobacter bau-mannii, analysis the drug-resistance mechanism and coping strategies, thus we can establish a new direction of clinical medication.Method The object of our investigation is Children's hospital in Tianjin.we ta-ke a systematicness review to the isolating and drug-resistanting information ofAcinetobacter(isolated between 1 January 2005 and 31 December 2006) and A-cinetobacter baumannii(isolated between 1 January and 1 March 2007) in thehospital. it include isolating information of Acinetobacter, pathogeny distribution, d-rug resistance to commonly used antibiotics of Acinetobacter baumannii. Datawere analyzed by using the SPSS10.0, the chi-square criterion was used to testfor the significances of association.Result①One handred and fifty-three strains of Acinetobacter were isolated fro-m linical specimens over the last four years(from 1 January 2003 to 31 Decem-ber 2006). There are 6 strains of Acinetobacter were isolated in 2003, 6 strainsin 2004, 17 strains in 2005, 124 strains in 2006. Ninety-four strains of Acinetob-acter baumannii were isolated between 1 January 2005 and 1 March 2007.②A-cinetobacter baumannii was isolated from 94 patients in urine(48.94％), sputumand throst mucus(32.98％), liquor puris(17.02％), dental ulcer swab(1.06％).③The r-esistant rate of Acinetobacter baumannii to cefalotin was the highest(100％), foll-owed by nitrofurantoin(90.43％), ampicillin(86.17％), cefoxitin(86.17％), amoxicillin/clavulanic acid(45.74％), and to other antibiotics all lower than 15％. There are t-wo MDR-Ab be found in 2006.④There was no change in the resistant rat-e compared with 2005 and 2006.⑤Four of these patients were granulocytopeni-a, two of them are leukemia, one of them is immune deficiency, seven of themuse ACH, nine of them use breathing machine, forty of them use urethral cathete-r.Conclusion①The infect rate of Acinetobacter especially Acinetobacter bauma-nnii in Children's hospital in Tianjin was increased markedly in recent years.②The resistant rate of Acinetobacter baumannii was showing a tendancy to inc-rease.③We have detected MDR-Ab in Children's hospital in Tianjin, especially resistant to imipenem, We must pay close attention to it.④The hospitalizedchildren who has immunefunction damaged and to accept invasion treatmentmust more caution the intection of Acinetobacter.⑤Rational use antibiotics is th-e key point of reduce the multidrug resistant Acinetobacter baumannii.In short, Acinetobacter baumannii is a major emerging nosocomial opportunistic patho-gen in hospital, which shows increasing drug resistance and even multiple drugresistance with important clinical impact. The monitoring to Acinetobacter bau-mannii is very important.
