Objective To investigate the pristine factors that can determine the rehabilitation of life and function for first hemiparetic stroke patients within a series of standardized scales. Methods 118 stroke subjects were registered prospectively. The Barther index(BI), Rankin scale(RS), Mortricity index (MI) , Mini-mental state examination (MMSE), Montgomery- Asberg depression scale(MADRS) and a scale of general state and risk factors were evaluated at the fourty-eighth hour, the fifteenth day and the ninetieth day after stroke. The three layers contain clinical factors (motor function, linguistic function, mental trait and psychological characteristics), functional factor (activity of daily living) and society factor (quality of life and family burden). Results The Barther index(BI), Rankin scale(RS) and Montgomery- Asberg depression scale(MADRS) changed significantly at the fourty-eighth hour, the fifteenth day and the ninetieth day after stroke (P<0.05).Between the fourty-eighth hour and the fifteenth day, the index of the three scales was no significant. But at the ninetieth day, the activity of daily living and the depression status was recoverd significantly. As shown by logistic regression analysis, the influence of activity of living at the ninetieth day after stroke include negative factors as pneumonia , anischuria at fourty-eighty hour , deep sensation disturbance and protective factors as strengths of extremitas superior at fourty-eighty hour, mimi-mental state examination at the fifteenth day. Conclusions The three layers that include clinical factors (motor function, linguistic function, mental trait and psychological characteristics), functional factor (activity of daily living) and society factor (quality of life and family burden) were improved significantly. The main factors influencing the activity of daily living of first hemiparetic stroke patients at the ninetieth day include pneumonia , anischuria and strengths of extremitas superior at fourty-eighty hour, mimi-mental state examination at the fifteenth day and deep sensation disturbance.
