Objectives To analyze the relationship between the severity ofOSAHS, cognitive function, sleepiness, BMI,serum insulin-like growthfactor-1 (IGF-1) and brain-derived neurotrophic factor(BDNF) in thepatients with obstructive sleep apnea hypopnea syndrome(OSAHS), andobserve the influence of nCPAP treatment on cognitive function, serumIGF-1 level and serum BDNF level.Probe into the cognitive impaireamong OSAHS patients and related influence factors.Design Cross-sectional study.Setting Sleeping lab, Department of Geriatrics, Second XiangyaHospital, Central South University, Changsha, ChinaParticipants 20 patients (AHI<15) and 39 patients withOSAHS(AHI≥15) were recruited.Method After polysomnography(PSG) all subjects were assessedfor congnitive function using the Mini-Mental StatusExamination(MMSE) and draw clock test, blood was drawned and serumIGF-1 and BNDF were determined by enzyme-linked immunosorbnentassay.14 patients with OSAHS (AHI≥15) were conducted nCPAPtreatment.The associations between the baseline characteristics, eachdomain and the total score of MMSE, IGF-1, and polysomnographic parameters were examined by bivariate correlate analysis and stepwisemultiple regression analysis.Results The differences of AHI, LSaO2, MSaO2, the total score ofMMSE, ESS and FOSQ and serum IGF-1 and BNDF level had statisticalsignificance between the two groups of the individuals. After 4 daysnCPAP treatment, above-mentioned all items were significantly improved.IGF-1 showed significant correlation with LSaO2. Stepwise multipleregression analysis selected BMI and BDNF as independent factors forpredicting the MMSE score. Stepwise multiple regression analysisselected L SaO2 and BDNF as independent factors for predicting theIGF-1.Conclusions The cognitive function, serum IGF-1 level and serumBDNF level were markedly impaired by OSAHS. They can be improvedby nCPAP short-term treatment. BDNF and BMI are independent factorsfor cognitive function. L SaO2 is the independent factor for predicting theIGF-1.
