Objective To evaluate the effects of propofol and IPC of the dynamic changes of alveolar apoptosis in ischemia-reperfusion injury in rat lungMethods 84 healthy male SD rats weighing 250-300g were randomly divided into 6 groups:①Sham group(n=6,suture was only placed in hilum of right lung.The other operation progress and analysis of species were same to IR group),②Ischemia group(n=6,only ischemia in right lung,which are done without any opreations),③Ischemia-reperfusion group(n=18,according to time,it is further divided into 3 sub-groups,which ischemia for 1 hour and stands for 1h,2h,4h after reperfusion),④ischemic preconditioning group(n=18,occlusion right lung 5 minutes and reperfusion 5 minutes,to repeat 3 times.The other operation progress and analysis of species were same to IR group⑤propofol treated group(n=18,3 sub-groups as group③).⑥propofol treat and ischemic preconditioning group(n=18,3 sub-groups as group③)We use the rat model of warm ischemia and repefusion in situ in single lung.The alveolar apoptosis index was achieved by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling(TUNEL).The expression of Caspase-3 in lung tissue was accessed by quantitative immunohistochemistry.And meanwhile,morphological and ultrastructural changes of lung tissue were observed by either light microscopy or transmission electron microscopy.Results①electron microscope result:Compared with shame group,ischemia group showed no changes and propofol treat and pretreatment group had much more changes than other groups②The expression of caspase-3:Compared with sham group,the expression of caspase-3 in group ischemia-reperfusion increased obviously(p<0.05).Propofol treat and ischemic preconditioning group could attenuate obviously this increase in caspase-3.(p<0.01)③The TUNEL result:There were no significant apoptosis changes in sham group.Ischemia reperfusion significant increased apoptosis index of lung.While propofol and ischemic preconditioning significantly attenuated these changes induced by ischemia reperfusion of lung.(p<0.01)④The expression of caspase-3 positively correlated to apoptosis.Conclusions①Apoptosis of lung participate in the injury induced by ischemia reperfusion of lung.②Propofol and ischemic preconditioning can ameliorate pulmonary IR injury and inhibit alveolar apoptosis,one important role may be it is down-regulation on Caspase-3 expression in lung.
