Serum Adiponectin Levels In Hypertension Patients With Obstructive Sleep Apnea-hypopnea Syndrome

ObjectiveTo explore the changes of serum adiponectin, tumor necrosis factor-α(TNF-α), and interleukin-6(IL-6)levels in hypertension patients with obstructive sleep apnea-hypopnea syndrome, and nasal continuous positive airway pressure (nCPAP) therapy.MethodsFollowing blood pressure and polysomnographic examination, 80 adult male patients were divided into 4 groups: 20 without hypertension or OSAHS, 20 with hypertension, 20 with OSAHS, and 20 with hypertension and OSAHS. There was no significant difference in age and body mass index (BMI) among the 4 groups. The serum levels of adiponectin, TNF-α,and IL-6 were measured by enzyme linked immunosorbent assay(ELISA). 20 patients with hypertension and OSAHS were treated by nasal continuous positive airway pressure (nCPAP) for one month, serum levels of white adipose tissue hormones were measured in these 20 patients 1 month after nCPAP therapy.ResultsThe serum TNF-αand IL-6 levels(52.74±5.47,60.85±5.21) in hypertension and OSAHS group was highest in 4 group and the serum adiponectin level(4.08±0.31) in hypertension and OSAHS group was lowest in 4 group.TNF-αlevel were significantly positively correlated with BP,DBP,AHI,T-SaO2<90% and significantly negatively correlated with MeanSaO2,MinSaO2; Serum IL-6 level was significantly positively correlated with SBP(r=0.568 P<0.001),DBP(r=0. 587 P<0.001),AHI(r=0.688 P<0.001),T-SaO2<90%(r=0.539 P<0.001) and significantly negatively correlated with MeanSaO2 (r=-0.733 P<0.001),MinSaO2(r=-0.733 P<0.001). Serum adiponectin level was significantly positively correlated with MeanSaO2 (r=0.686 P<0.001),MinSaO2(r=0.730 P<0.001) and significantly negatively correlated with BMI(r=-0.259 P<0.001),waist circumsference(r=-0.451 P<0.001),T-SaO2<90%(r=-0.524 P<0.001),AHI(r=-0.644 P<0.001),SBP(r=-0.579 P<0.001),DBP(r=-0.633 P<0.001),TNF-α(r=-0.957 P<0.001),IL-6(r=-0.938 P<0.001). After one month's treatment of nCPAP, there was a significance decrease in serum level of TNF-αand IL-6 and a significance increase in serum adiponectin level in hypertension and OSAHS group.ConclusionsThe serum levels of TNF-αand IL-6 in hypertension group or OSAHS group were significantly higher than those in the control group,and they were highest in hypertension and OSAHS group.The serum level of adiponectin in hypertension group or OSAHS group was lower than those in the control group,and it was lowest in hypertension and OSAHS group. Effective nCPAP therapy might decreased serum TNF-αand IL-6 levels and increased plasma adiponectin levels in hypertension and OSAHS group,then reverse the associated target organ damage.
