AIM:To study the expression of bFGF,FGFR-2 and Hpa in the H pylori infected gastric mucosal epithelia and its significance in the carcinogenesis of stomach.METHODS:A total of 30 patients with chronic superficial gastritis(CGS),29 with intestinal metaplasia(IM),31 with dysplasia(Dys)and 55 with gastric carcinoma(GC)were included in this study,Expression of bFGF,FGFR-2 and Hpa were assessed immunohistochemically(SP method).Rapid urease test and histological examination with Warthin- Starry were used to assess the status of H pylori infection.RESULTS:The expression of bFGF and FGFR-2 in CSG group were significant lower than that in other three groups(vs IM group:P<0.05;vs Dys group:P=0.000;vs GC group:P=0.000),the expression of bFGF and FGFR-2 in IM group were significant lower than that in Dys and GC groups(vs Dys group:P<0.05;vs GC group:bFGF P<0.01;FGFR-2 P<0.05),but there was no significant difference between Dys and GC groups.The expression of bFGF and FGFR-2 were significantly higher in H pylori infection group than that in the group without in IM group(P<0.01)and Dys group(P<0.01).The expression of bFGF and FGFR-2 in H pylori infection Dys group were significantly higher than that in GC group(P<0.05),while no significant difference was found between GC and IM groups.The expression of bFGF in Dys and IM groups without H pylori in fection were significantly lower than that in GC group(vs Dys group,P<0.05;vs IM group,P=0.000).The expression of FGFR-2 in H pylori-negative IM was significant lower than that in GC(P<0.05),while no significant difference was found between H pylori-negative Dys and GC groups. The heparanase expression in gastric carcinoma was significantly higher than that in the other three groups,but no significant difference was shown between CSG,IM and Dys groups;in H pylori positive group,the expression of Hpa showed no significant difference among IM,Dys and GC groups,but significantly higher as compared with that in CSG group(vs IM,Dys groups P<0.05;vs GC group P<0.01).The invasive depth(bFGF group P<0.01; FGFR-2 and Hpa group P<0.05),lymph node involvement(P<0.01),distant metastasis(bFGF,FGFR-2 groups P<0.01;Hpa group P<0.05)and TNM stage (P<0.01)of gastric carcinoma had relationship with the overexpression of bFGF,FGFR-2 and heparanase,while sex,age and histological differenciation grade did not.CONCLUSIONS:The overexpression of bFGF,FGFR-2 and heparanase are involved in the carcinogenesis and malignant progression of gastric carcinoma.And H pylori infection may play an important role in the carcinogenesis of stomach through up-regulation of the expression of bFGF, FGFR-2 and heparanase.
