Objective:Liver transplantation(LT)is the effective treatment for hepatocellular carcinoma(HCC),however,prognostic factor and selection of patients with HCC for LT are controversial.Identify prognostic factors can contribute to establish a patient selection criteria and improve tumor-free survival.Method:The clinical and follow-up date of 43 patients who underwent LT for HCC between Mar.2003 and Apr.2006 in our center were analyzed retrospectively.Their survival rate were calculate by Kaplan-Meier method. Log-rank test and Cox propotional hazards regression model were performed to analyze prognostic factors of HCC after LT.All statistical evaluations were performed using SPSS 13.0 statistical software.Results:The 0.5-,1-and 2-year survival rates were 97.7%,88.1%,77.4% respectively.The rate of disease-free survival were 88.4%,69.1%,69.1% respectively.Univariate analysis revealed that alpha-fetoprotein(AFP), presence of vascular invasion,initial onset and recurrence of hepatocellular carcinoma,adjacent tissue invasion in abdominal cavity,postoperative chemotherapy,the tumor size,combined action of the size and number of tumor were prognostic factors of HCC after LT(P<0.05).Cox regression analysis suggested adjacent tissue invasion in abdominal cavity was an independent prognostic factor of survival rate and disease-free survival (P<0.05),vascular invasion was a factor of disease-free survival(P<0.05).Conclusions:vascular invasion was an independent prognosis factor of disease-free survival.Adjacent tissue invasion in abdominal cavity was an independent prognostic of survival rates and disease-free urvival.
