It's the physiological change about cellular immune function in parent that sustains the development of pregnancy in gestational period, which would not only increase the susceptibility to causative organism simultaneously but also to result in the augmentation about infection-dependent morbidity and mortality. On one hand, bacterial vaginosis or chorioamnionitis makes chief contribution to premature rupture of membrane. On the other hand, the natural anti-infection immunologic system in pregnancy women will be activated at the same time. Infection immunity is composed of inflammatory corpuscle, extracellular matrix as well as cytokine all of which are important ingredients, not a single one can be omitted. The secretions from lateral wall of vagina and cervical canals were collected for routine examination and the culture of UU and CT randomly from 105 pregnant women. Then specimens of fetal membrane were taken after delivery including normal pregnancy women and those complicating PROM for random in order to analyse the expression and significance of CA, laminin known as the important structure of basement membrane andα-subunit of receptor of interleukin-2 belong to Th1 type cytokines in the first section of study. We arrived at such conclusions as follows: The women with PROM have more tendency to occur preterm delivery. There were significant relationships between PROM and CA or BV.Not only can infection or inflammation attenuate the structure of fetal membrane,but also the pregnant women's immunologic responses to infection are able to disrupt the fetal membrane.Furthermore,infection immunity seems to be a trigger of labour,sometimes even to induce premature delivery.At present, conservative management after spontaneous membrane rupture defined as treatment directed at continuing the pregnancy add the economic and psychological burden among women with PROM even to trouble their daily lives.In order to avoid such disadvantages,12 cases complicating PROM near term without intrauterine infection were choosed strictly according to inclusion criteria to take part in the initial clinical application of intracervical blockage in section II of the study . The conclusions are that intracervical blockage is able to make up a physical and chemical barrier upon the cervix uteri of whom suffering PROM without trauma. There are neither ischemic change on the distal end of cervix uteri obviously nor the spilling of amniotic fluid again via cervix uteri ,especially , there are no need to circumscribe the body position and daily lives of the pregnant woman.
