Objectives: To observe expression of PAR1 in perihematoma tissue of IntracerebralHemorrhage(ICH) patients; so as to investigate into the relationship between theexpression of PAR1 and Sthenia ZHENG, Protration ZHENG, as well as therelationship among hematoma volume and the scores of nervous impairment, toinquire PAR1 in delayed neural injury following ICH and then to provide materialfoundation for the microcosmic syndrome differentiation with TCM to quest for newtreatment.Materials and methods: 22 patients with ICH were collected and divided into threegroups: YIN sthenia group, YANG sthenia group and protration ZHENG group. Allwere accord with the diagnosing point of ICH and must be able to be diagnosed asone of the syndromes based on the differentiation standards of TCM and undergonesurgical operation, among which 17 were male and 5 were female. The average agewas 55.68±13.36 years. According to the time from the onset of the disease to beoperated, the 22 patients were divided into the following groups:<6hour(n=6),6-12hour(n=8),12-24 hour(n=5) and>24hour(n=3), to collect the brain tissue to take aspathological specimens,trail team included the brain tissues close to hematoma about1.0cm and control team included relatively normal brain tissues far from hematomaexceed 2.0cm on the surgical route. The standard of nervous impairment of acutecerebral accident was used to score, the DUOTIAN formula to calculate thehemorrhage volume, the degree of brain tissure damage was reflected by HE staningmethod,and the expression of PAR1 was detected with immuno-histochemistymethod.Results:1. HE staning showed that preihematoma tissure was almost normal in the controlgroup and<6h hours group, tissure damage was mild in the 6-12 hours group, moresevere in the 12-24 hours group and severe>24 hours group.2. Immuno-histochemisty staning showed that the expression of PAR1 in theperihematomal brain region of ICH patients significantly increased in comparison with the normal control groups, 5.37±3.72 and 0.92±0.43 respectively (P<0.01). Therewere no obvious difference of the expression of PAR1 between<6h hours groupand the normal control group, the expression of PAR1 upregulated 6 hous after theonset of ICH and had the trend of increasing gradually.3. The expression of PAR1 were significantly different among YIN sthenia, YANGsthenia and the protration ZHENGs,were 2.30±1.57,5.15±2.80 and 9.76±2.33respectively (P<0.01 or P<0.05), The expression of PAR1 of protration ZHENG wasthe highest, YANG sthenia second, YIN sthenia last;4. The course of diseases were not obviously different among YIN sthenia, YANGsthenia and the protration ZHENGs (P>0.05). The hematoma volume and the scoresof clinical nervous impairment were significant different among the three groups inICH(P<0.01). The volume and the scores of protration was the first, YANG stheniasecond, YIN sthenia last.5. There were positive correlation among the expression of PAR1, hematomavolume and the scores of clinical nervous impairment(P<0.01). There were positivecorrelation between The expression of PAR1 and the course of diseases (P<0.01).There were no correlation between the course of diseases and hematoma volume, thescores of clinical nervous impairment.(P>0.05).Conclusion:The expression of PAR1 may be involved in the delayed brain injury after ICH,the expression of PAR1 in the perihematomal brain region of ICH patients couldserve as a microcosmic index to judge the difference between stroke excess and strokeprostration. According to the study we attempt to treat ICH with medication ofsuppressing the expression of PAR1 to improve the ICH patient curative effect.
