Objective: The study attempted to investigate psychosocial factors, family functionof primary insomnia patients, and researched the mechanism of neuroendocrine ininsomnia, and observed the curative effect of cognitive-behavioral therapy to insomniacs.Methods: 70 patients with primary insomnia and 60 normal controls matched by ageand sex were evaluated with following questionnaires: general condition questionnaire,PSQI, SCL-90, SCCS, FACESâ…¡-CV, LSIB, and measured blood cortisol. The studyanalyzed the differences between the two groups. 64 insomniacs taking BZD long termwere allocated to a drug group and a combined group (received drug gradual tapering plusCBT). All subjects measured all of the above questionnaires, and the study compared thecurative effect of the two groups.Result: (1) The score of SCL-90, SCCS and LSIB in insomniacs notably differedfrom that of normal controls. (2) There were correlations between PSQI and gender,economic state, educational level, family mode. (3) The score of FACESâ…¡- CV ininsomniacs differed from that of normal controls. The rate of balance type family ininsomniacs was lower than that of the controls; the rate of extreme type family ininsomniacs was higher than that of the controls. (4) The level of blood cortisol ininsomniacs was higher than that of the controls. (5) The score of family cohesion and LSIBwere negative correlation with PSQI; the score of SCCS, disaffection A and the level ofblood cortisol were positive correlation with PSQI. (6) The score of SCL-90, SCCS,FACESâ…¡-CV and LSIB in CBT group notably differed from that of drug group. (7) Boththe improvement of sleep quality and the rate of clinic obvious curative effect in CBTgroup were better than that of the drug group. (8) The average dosage of BZD in CBTgroup was notably less than that of the drug group.Conclusions: (1) Female, mixed-dwelling, high levels of economic state andeducational background were risk factors of insomnia. (2) There were more questions onpsychological and personality and lower life satisfactory in insomniacs than that of healthy people. (3) There were lower family function and more extremely family type ininsomniacs than that of healthy people, which was disadvantage to insomnia. (4) The levelof blood cortisone in insomniacs was higher than that of healthy people, which wasnegatively correlated with the sleep quality and daytime function. (5) The mental healthand sleep quality of insomniacs were improved after CBT, and the clinic curative effect ofinsomniacs received CBT was better than that of insomniacs received only drugs. (6)Insomniacs could reduce the dosage of BZD by received CBT.
