Objective: To investigate the expression of CD40 and BCL-2 in bladder transitional cell cancer, To explore respectivly the correlation of it with clinicopathological paramerers(clinical stage, pathological grades,et al) and mutual relationship.To measure the apoptosis rate in bladder transitional cell cancer.The relationship between CD40﹑BCL-2 and apoptosis was analyzed.Methods:Expression of CD40 and Bcl- 2 were evaluated by immunohistochemistry in bladder transitional cell cancer(78 cases). Apoptosis of bladder transitional cell cancer was detected by Hoechst to determine the apoptotic rate (AR), simutulatiously in accordance with 20 normal bladder transitional urothelium of donor. Results:1.The expression rate of CD40 in the cases of normal bladder tissue was 1/20(5%),which in the cases of bladder transitional cell cancer was 55/78(70.5%),CD40 was highly expressed in bladder transitional cell cancer.A statistically significant differences of CD40 staining were observed between normal bladder tissues and bladder transitional cell cancer(p<0.01).Overexpression of CD40 was strongly inversely related to the tumor grade(p<0.05)﹑ clinical stage(p<0.01)and lymph node metastasis(p<0.01).2. The expression rate of BCL-2 in the cases of normal bladder tissue was 1/20(5%),which in the 78 cases of bladder transitional cell cancer was 36/78(46.7%),A statistically significant differences of BCL-2 staining were observed between normal bladder tissues and bladder transitional cell cancer(p<0.01). Expression of BCL-2 was positively correlated with the tumor grade(p<0.05)﹑ clinical stage(p<0.01),and lymph node metastasis(p<0.05).3. AR of bladder transitional cell cancer was 10.64士0.63, the positive group of CD40 was 12.60士0.38, the negative group of CD40 was 6.77士0.53, A statistically significant differences of apoptosis were observed between CD40 positive group and CD40 negative group(p<0.01),the expression of CD40 was positively correlated with AR; the positive group of BCL-2 was 7.01士1.13, the negative group of BCL-2 was 11.03士0.62, A statistically significant differences of apoptosis were observed between BCL-2 positive group and BCL-2 negative group(p<0.01),the expression of BCL-2 was inversely correlated with AR.Conclusion:1. CD40 was involved in development of bladder transitional cell cancer.2. CD40 was inversely related with clinical stage,phthological grade and lymph node metastasis, it indicated CD40 probably became a valuable marker to evaluate malignant degree,metastasis of lymph node and evaluation of prognosis.3. CD40 inhibited the development of tumor through precipitating apotosis, CD40 might be a useful therapeutic target for biological therapy of bladder cancer.4. BCL-2 was involved in development of bladder transitional cell cancer.5. BCL-2 was posively related with clinical stage,phthological grade and lymph node metastasis,it indicated BCL-2 probably became a valuable marker to evaluate malignant degree,metastasis of lymph node and evaluation of prognosis.6. BCL-2 precipitated the development of tumor through inhibiting apotosis.7. The expression of CD40 was inversely correlated with the expression of BCL-2,It showed CD40 might induce cell apotosis with downregulation the expression of BCL-2.
