Objective:To investigate the expression of matrix metalloproteinase-1 (MMP-1)and tissue inhibitor of metalloproteinase-1(TIMP-1)in the pathogenesis of pulmonary hypertension due to ventricular septal defect.Methods:20 ventricular septal defect(VSD)patients suffered from congenital heart disease(CHD)were selected randomly and divided into 2 groups according to Heath-Edwards pulmonary vessel pathologic grading:there were 12 cases in group 1 characterized by PH(gradeⅠ(n-6),gradeⅡ(n=6)),and 8 cases in group 2 not characterized by PH.RT-PCR was applied to detect the mRNA of MMP-1 and TIMP-1 in the lung tissue of these patients.SPSS 11.5 was operated for statistics analysis.Results:(1)The expression of the MMP-1 mRNA of the PH group(PHⅠ,Ⅱ)is obviously more than of the no PH group,P<0.01;the expression and the pathologic ratings(no and PHⅠ,Ⅱ)become obviously related,r=0.737,P<0.01.(2)The expression of the TIMP-1 mRNA of the PH group(PHⅠ,Ⅱ)is obviously more than of the no PH group,P<0.01;the expression and the pathologic ratings(no and PHⅠ,Ⅱ)become obviously related,r=0.866,P<0.01.(3)The TIMP-1 mRNA/MMP-1 mRNA specific value and the pathologic ratings(no and PHⅠ,Ⅱ)become obviously related,r=0.698,P<0.01.(4)The expression of the MMP-1 mRNA and the TIMP-1 mRNA and the TIMP-1 mRNA/MMP-1 mRNA specific value all become unrelated with ages of patients,r=0.052,0.270,0.2990,all the value of P greater than 0.05.Conclusion:(1)The expression of the MMP-1 mRNA and the TIMP-1 mRNA of the PH set(PHⅠ,Ⅱ)is obviously more than of the no PH set;and their expression and the TIMP-1 mRNA / MMP-1 mRNA specific value all become obviously related with the pathologic ratings(no and PHⅠ,Ⅱ).Suggestion above is the increased expression the MMP-1 and TIMP-1 as well as the unbalance of the MMP-1/TIMP-1 may be the contributor of the pathologic process of pulmonary hypertension(PHⅠ,Ⅱ)due to ventricular septal defect,in which pulmonary vascular remodeling were affirmed the important turning point.(2)This hints as a result,decreasing the expression of MMP-1 and TIMP-1 and keeping the MMP-1/TIMP-1 balance may repress the progress of pulmonary vascular remodeling,prevent from thus PH occurrence and becoming aggravated, and profit the sufferer in future.
