Objective To approach the level and variation of serum matrix metalloproteinase(MMP-1,MMP-2),metalloproteinase inhibition(TIMP-1)and the rate of matrix metalloproteinase and metalloproteinase inhibition (MMP-1/TIMP-1,MMP-2/TIMP-1)and assess the relation of them and the pulmonary hypertension(PH)in patients with congenital heart disease.Methods According to the preoperative pulmonary arterial systolic pressure(SPAP),81 patients with congenital heart disease were divided into four groups:non PH group(16 patients,SPAP<30mm Hg);light PH group:(21 patients,30mmHg≤SPAP<40mmHg),moderate PH(21patient,40 mmHg≤SPAP<70mmHg)and heavy PH(23 patient,SPAP≥70 mmHg).And the variation of these indexes would be observed in 19 patients among the moderate PH and heavy PH group were comprehensively treated with PGE1 et al.They also were examined before and after 24 patients of the total patients had be operated.The concentrations of serum MMP-1,MMP-2 and TIMP-1 were measured with ELISA.A control group was made up of 21 unimpaired children.Results observation group MMP-1,MMP-2,TIMP-1 and MMP-2/TIMP-1 and the comparison group compares remarkably advances,but the MMP-1/TIMP-1 change is not remarkable;After has further carried on the grouping analysis discovery to the observation group,the observation group whether there is regardless of does merge pulmonary hypertension MMP-1, MMP-2 comparatively compares the group remarkably to advance,TIMP-1 advancing compares after the merge pulmonary hypertension with the comparison group has statistics difference,is not remarkable in the non hypertension group TIMP-1 change;As well as is light,moderate pulmonary hypertension group MMP-2/TIMP-1 in the non- pulmonary hypertension group and the comparison group quite even remarkably advances, but compares the non- statistics difference in the specific heavy pulmonary hypertension group MMP-2/TIMP-1 change and the comparison group;Whether there is the observation group regardless of does merge the pulmonary hytertension MMP-1/TIMP-1 change and the normal comparison group compares does not have statistics difference.19 examples after PEG1,intake nitroglyserin on treatment 14 day before MMP-1,MMP-2 and TIMP-1 and the treatment compare have the remarkable drop,but MMP-2/TIMP-1 drops not remarkably.,24 examples treat 7 day after the surgery,MMP -1,MMP-2, TIMP-1 and MMP-2/TIMP-1 the technique compare remarkably reduces,the difference has statistics significance.MMP-1 and MMP-2,MMP-2andTIMP-1, MMP-1 and MMP-1/TIMP-1,MMP-2 and MMP-2/TIMP-1 assume the remarkable correlational dependence.TIMP-1 and MMP-1/TIMP-1,TIMP-1 and MMP-2/TIMP-1 assume the remarkable negative relations.Conclusion(1)congenital heart disease whether have or not pulmonary hypertension blood serum to have MMP-1,MMP-2 remarkably advances, MMP-1,MMP-2 participation pulmonary vascular remodeling entire process.
