Objective: To compare the postoperative complications, incidence rate of the two operative methods—subfascial dissociate and subperiosteum dissociate in the pterion approach. Methods: Evaluate the clinical effection of the postoperative complications happened on the 75 patients from 2005 to 2006 which have taked cranium-opened operations, using the methods of subfascial dissociate or subperiosteum dissociate in the pterion approach retrospectively and analysis it statistically. Results: Of the 30 patients using subfascial dissociate, 1 suffered from postoperative facial palsy,none suffered from temporal amyotrophy; of the 45 patients using subperiosteum dissociate, none suffered from postoperative facial palsy, 1 suffered from temporal amyotrophy. According to Physhal analysis,both groups have no significant deviation between the injury of facial nerve and temporal amyotrophy (p>0.05) .The method of subperiosteum dissociate can faster than the methods of subfascial dissociate. Conclusion: In the pterion approach, the method of subperiosteum dissociate can protect the frontal branch of the facial nerve effectively ,and it has the feature of relative convenience, shortcut and little trauma.it is applicable for some surgeries,especially the anterior circulative angiopathy, anterior cranial fossa' spathological changes, hypertensive cerebral hemorrhage.
