OBJECTIVE:This paper was focused on analyzing the factors of irrational antibacterials use inducing adverse drug reaction.We hope medical workers can change this situation and reduce ADRs and medical costs.METHODS:We use the retrospective analysis method and collect the reports on adverse drug reactions and cases.According to the guiding principles for clinical use of antibacterials,we analyze ADRs induced by irrational antibacterials use.In this study some factors such as gender,age,antibacterials,type of ADRs have been taken into consideration.We investigated the type and the amount of antibacterials used in hospital during 2005~2006 and determined the relationship between antibacterials use and adverse drug reaction.We analyze and evaluate the reports about ADRs indeced by irrational drug use based on the domestic pharmaceutical journal published from 2000 to 2006.RESULTS:In 1036 ADRs reports collected,there are 432 cases of ADRs induced by antibacterials,accounting 41.7%according to the standard of irrational use of antibacterials.We found 106 cases were induced by irrational drug use.The results show that 22 cases were due to the long time of medication,52 cases of rapid speed of infusion(including 36 cases possibly),8 cases of high drug concentration in infusion,11 cases of unknown allergic condition,4 cases of incorrect drug dose,4 cases of inappropriate combined medication,1 case of no allergic test,1 case of frequent change of drug use.We found 91 cases about ADRs induced by irrational drug use reported in the domestic pharmaceutical journal published from 2000 to 2006.54 cases were induced by the high drug concentration in infusion,10 cases by the rapid speed of infusion,8 cases by the long time of medication,7 cases by the incorrect drug dose,6 cases by unknown allergic condition,2 cases by inappropriate combined medication,2 cases by no allergic test and 2 cases by inappropriate route of administration.The amount of antibacterial drugs used in our hospital was in order decreasingly by cephalosporins,quinolones,β-lactamase inhibitor compound and penicillins.The amount of these four drugs covers over 77%of all drugs.And the rate of ADRs induced by antibacterials was in order decreasingly by quinolones, cephalosporins,β-lactamase inhibitor compound and penicillins.The rate of ADRs induced by these four drugs is over 80%.The positive correlation between the use of antibacteriais and ADRs was determined.CONCLUSIONS:In this study,we have found some influencing factors on the use of antibacterials and ADRs.There is a positive correlation between the use of antibacterials and ADRs.We also have studied the latest papers related to the situation of ADRs induced by irrational drug use,which are helpful for medical workers and patients to use antibacterials rationally,and to reduce ADRs and medical affaires;Meanwhile,it is meaningful to reduce the antibacterials resistance and the medical cost.
