ObjectivesGestational diabetes mellitus is a specil type of diabetes mellitus,and it is increasing with life level.Gestational diabetes mellitus flects metabolically alteres fetal environment.Studies in pima Indians revealed a U-shaped relationship between birth weight and risk of type 2 diabetes.Blood glucose level are markedly related with fetal growth and develepment.Sex hormone blinding globulin is a group of substance which related to IR.IGF-1 are closely related with the fetal growth and development,and IGF-1 and SHBG are regulated by Ins.We study new borns'umbilical cord for GDM and control group.we determine the SHBG,Ins,IGF-1,FBG及IR of all the specimens in order to investigate these indexs which change in the borns'umbilical cord of their newborns。and we want to know how these indexs effect the new borns 'growth and regulate glucose level.methods1.Subjects:We collected 30 cases newborns of pregnancy women who were definitely diagnosised as GDM for study group.,58 cases newborns of pregnancy women who were no GDM and without any complications as control.matched for age,gestational age,gestational numbers,study group and control group are divided into 5 groups:<2500g,2500~2999g,3000~3499g,3500~3999g,≥4000g;and they are divided into 3 groups:<2500g as SGA,2500~3999g as AGA,≥4000g as LGA。 2.specimen collection:we were able to collect umbilical cord blood 5 ml from 58 cases newbores.The cord blood were withdrawn from cord vein immediately after delivery of late pregnancy.The Serum of the newborns'umbilical cord were stored at -25℃untill assayed.Only dates of birth weight and length.The test used ELISA meas to detect the SHBG and IGF-1 concentration in the umbilical cord serum of two groups newbores.Blood glucose were measured by glucose-oxidase assay,Ins in the umbilical cord serum were measured by Electrochemical luminesecne.3.Statistical analysis All data was assayed by SPSS for windows 11.5 and presented as medians with ranges.Sample mean were compared with student t test.Correlation was calculated by the spearson rank correlation.Results1.According birth weight,two groups newborns are divided into 5 groups.Comparesion between the GDM group and the control group shows differences the levels of SHBG in the umbilical cord serum of two groups(P<0.01).Comparesion between the GDM group and the control group shows differences the levels of IGF-1 for the umbilical cord serum in 3500-3999g and 4000g 'groups of birth weight (P<0.05);other study groups are significant higher than control groups.(P<0.01).the the IR's level of study groups are significantly higher than control group for birth weight≥4000(P<0.05);other study groups are significant higher than control groups. (P<0.01).2.The SHBG,IGF-1 level is positive correlated with birth weight(γ=0.870,γ=0.904,P<0.01.).The levels of IR,Ins of cord blood of newborns in GDM is nagenitive correlated with birth weight(γ=-0.547,γ=-0.745,P<0.01).The levels of IR,SHBG,IGF-1,Ins and FBG of cord blood of newborn In control group is positive correlated with birth weight,r=0.534r=0.824,r=0.935,r=0.418 and r=0.571;P<0.01.3.According birth weight,two groups newborns are divided into 3 groups.The levels of SHBG and Ins of cord blood of SGA newborns in ODM group were esignificent higher than those for cord blood of AGA and LGA(P<0.01).conclusions1.There may be an obvioursly IR in newborns of ODM,and SGA of newborns is most significant.SHBG may be a mark what determine IR of ODM.2.The levels of SHBG and IGF-1 of cord blood of newborn of GDM is significent correlated with birth weight.3 Level of FBG,insulin,IGF-1 and IR of ODM at birth are in higher.4.IGF-1 plays a major role for fetus growth and development in utero.IGF-1 may increas fetus growth and development throng more secreting.
