Objective:1. To study the esophageal carcinoma patients' immune function and evaluate the relation between transcription factor T-bet/ GATA-3 and Th1/ Th2 type cytokines in peripheral blood from esophageal carcinoma patients,we detected the gene expression of Th1/Th2 type cytokines IFN-γ/IL-4 and transcription factors T-bet/GATA-3.2. Investigate the influence of immunotherapy on the expression of T-bet/GATA-3 mRNA in patients with esophageal carcinoma,in order to search new path to improve esophageal carcinoma patients' immune function.Methods:1. According to CT image,40 patients were divided into two groups,stage I plus II,and,stage III plus IV,The gene expression of T-bet/GATA-3 in peripheral blood monouclear cells(PBMCs) from 40 patients with esophageal carcinoma and 20 normal people was detected by reverse transcription-polymerase chain reaction(RT-PCR) .The expression of IFN-γand IL-4 for Th1/Th2 cytokine in blood plasma was detected by enzyme linked immunosorbent assay (ELISA).2. Forty patients with esophageal cancer were randomized divided into two groups,20 received radiotherapy plus Highly Agglutinative Staphylococcin (HAS) and 20 received radiotherapy alone.The expression of T-bet mRNA/GATA-3 mRNA,IFN-γ/IL-4 was measured by the same method as that just be mentionedabove in both two groups before treatment and after treatment.Results:1. T-bet mRNA and Th1 cytokine IFN-γexpression of patients with esophageal carcinoma was significantly lower than that in normal people,the expression of GATA-3 mRNA and Th2 cytokine IL-4 in esophageal carcinoma was significantly enhanced.However,the difference between two esophageal carcinoma groups was not significant.2. After treatment T-bet mRNA expression of both two groups was decreased,and GATA-3 mRNA expression was increased.However, After treatment the expression of T-bet mRNA in Study group was higher than in control group,and GATA-3 mRNA was lower than that in control group.Conclusions:1. In patients with esophageal carcinoma,we found there are lower production of IFN-γfor Th1 and over-production of IL-4 for Th2,which means there is imbalance between Th1 and Th2,esophageal carcinoma patients have a predominant expression of Th2 cytokines.However,the difference between two esophageal carcinoma groups was not significant.2. The expression of T-bet in patients were lower than control groups,but the results of GATA-3 were opposite.There was a positive correlation between the expression of T-bet and IFN-γ,The positive correlation between GATA-3 and IL-4 was also found in patients.It means that the imbalance differentiation of Th1/Th2 may correlate with the imbalance of T-bet/GATA-3,and also means that T-bet/GATA-3 play an important role in the development of esophageal carcinoma.3. Patients' immune function was repressed by radiotherapy,and this change can be relieved by Highly Agglutinative Staphylococcin (HAS) through increasing the expression level of T-bet mRNA.
