Background and objective:Cerebrovascular disease (CVD) is the most frequently fatal and disabled disease,while it is generally considered that the disorder of lipid metabolism is one of its most etiological factors. Among all sorts of lipids,the relationship between lipoprotein(a) [Lp(a)] and Cerebrovascular disease attracts increasingly attention. It is reported that the lipid leads to the atheromatosis in cerebralvascular through multiple mechanisms,they mainly promote large vessels atheromatosis rather than small vessels,which caused acute and choronic cerebrovascular disease. Through this research we want to detect the correlation between serum lipoprotein(a) level and Cerebrovascular disease [including cerebra infarction(CI), cerebral hemorrhage (CH), and subtypes of them] and some other dangerous factors of CVD in order to provide information for the prevention and cure of stroke .Methods:1.All objects were selected from the stroke patients at the neurology department of the first affiliated hospital of the Nan chang university from January 2006 to August 2006,and all of them got the first stroke invasion without taking hypolipidemic and anticoagulant drug, we have excluded patients with cerebral embolism, subarachnoid hemorrhage, liver, kidney diseases, tumor, blood and immunitym system diseases, who have addiction of alcohol and smoking ,drug abuse also ruled out. We noted the age, sex, job, the history of high blood pressure, diabetes , heart disease, scope of CI(large area ,medium area or lacunar ) ,volume of blood (serious or not), distribution of stroke vessels (anticirculation or postcirculation), the control group is selected from healthy adults examined by doctors at the same period.2.We drawed patients'empty stomach venous blood in the second morning , serum lipids concentration were assayed by enzymatic turbidimetric immunoassay and enzyme-linked immunosorbend. Use Hitachi 7600 machine to detect the samples. 3. All data were analysised by SPSS11.5 software.Results:1.The serum Lp(a) level of CI patients, CH patients and control group shows a obviously low concentration and skewness distribution curve.2.We find that the serum Lp(a) lipid level is significantly increased in CI patients rather than CH patients and control group(P<0.05),between CI and control group patients it has no statistical difference(P>0.05).3.Among all subtypes of CI ,the Lp(a) level is significantly increased in large and medium vessels groups than in control group (P<0.05),the data of lacunar infarct and control group patients has no statistical difference(P>0.05).4.The Lp(a) level of all subtypes of CH(large ,common,merely cerebroventricular haemorrhage group) has no statistical difference(P>0.05) compare to control group (P>0.05).5.The Lp(a) level of the anti-circulation and post-circulation group of CI is significantly increased rather than control group(P<0.05),but the difference between them has no statistical difference (P>0.05). the difference among anti-circulation, post-circulation and cerebroventricular haemorrhage group compare to control group has no statistical difference (P>0.05).6.Multiple linear regression analysis shows no statistical difference among Lp(a) level with age,sex,job,disease history of high blood pressure,diabetes ,heart and other lipids (TC,TG,LDL,HDL) (P>0.05 ).7.Logistic regression analysis shows high level of serum Lp(a) is an important dangerous factor to ischemic stroke.Conclude:1.The high level of serum Lp(a) is one of the important dangerous factors to ischemic stroke ,especially to large and moderate vessels, the systems of basal artery and cervical internal carotid artery both can be sufferred ,it has no statistical difference compared to lacunar infarct and CH.2.The serum level of Lp(a) is independent to age, sex ,job ,disease history of high blood pressure,diabetes ,heart and other lipids.3. People's serum Lp(a) level shows a low concentration and skewness distribution curve.
