Objective: to observe the inhibiting effect of Ketoconazole and Diaethylstilbestrol on penile erection after urethroplasty for hypospadias.Patients and Methods: Twenty-eight patients who underwent urethroplasty for hypospadias were divided into group A,B at random. Patients in group A took Ketoconazole, and patients in group B took Diaethylstilbestrol a day before operation and three days after operation. The dosage of Ketoconazole for adult and child with more than 30kg was 400 mg, three times every day, and for child with less than 30kg was 200mg, three times per day. The dosage of Diaethylstilbestrol for patients was 1mg, three times per day. The level of testosterone and glutamyl-pyroracemic–transferase (GPT) andγ-glutamyl–transferase (γ-GT) in blood serum was determined before and after operation. Furthermore, the frequency and duration of penile erection,times of painful erection in night,erection in early morning,bleeding after operation and nocturnal penile tumescence were recorded.Results: The rate of effective erection-inhibiting was 92.9% in group A, while 42.9% in group B. The difference was significant between the two groups (p<0.05). The concentrations of testosterone were 31.6±11.5ng/dl and 7.2±8.2 ng/dl before and after surgery for children (p<0.0001) in group A, individually. In 2 adult patients in group A, the mean concentration of testosterone decreased dramatically from preoperative 711.6 ng/dl to postoperative 203.4ng/dl. The level of GPT andγ-GT were normal in all patients except for one case with mild change in group B.Conclusion: Ketoconazole was more effective in inhibiting penile erection than Diaethylstilbestrol. The two drugs showed no adverse impact on hepatic function in short time.
