Locally Advanced Non-small Cell Lung Cancer 3-dimensional Conformal Radiotherapy Effect Prospective Study

Objective:To evaluate the therapeutic effect and toxicity of three-dimensional conformal radiotherapy (CRT) and sequential Gemcitabine , and Cisplatin chemotherapy in locally advanced , unresectable , StageⅢnon-small cell lung cancer (NSCLC).Methods:60 patients were divided randomly into the following groups : CRT group was given to a total dose of 68~80 Gy (4~6 Gy/ fraction 10 - 20 fractions) by using three-dimensional conformal radiotherapy techniques ,and conventional radiotherapy group was given to a total dose of 60~70 Gy (2 Gy/ fraction 30~35 fractions) . Both groups were given four cycles of sequential chemotherapy (Gemcitabine 1000 mg/ m2 on Days 1 ,8. Cisplatin 80 mg/ m2 on Days1~3) .Results:The response rate was76.7% , composed of 11 complete responders and 12partial responders for CRT group ,and 66.7 % ,4 , 16 for conventional radiotherapy group , respectively. The survival rate was 70 % at 1 year ,60 % at 2 years , the median survival time was 26 months for CRT group and compared with53.3% ,40 % ,17 months for conventional radiotherapy group , respectively. Grade 1-2 radiation esophagitis and pneumonitis occurred in 66.6 %(20/30) ,13.3 %(4/30) for CRT group patients , and occurred in 63.3 % ( 19/30) ,10%(3/30) for conventional radiotherapy group patients.Conclusion:Three-dimensional CRT and sequential chemotherapy ,as described here , is a well-tolerated regimen with acceptable toxicity . More effective treatment schemes are required to improve local disease control and overall survival.
