ObjectiveThrough the occupational health survey for oilfield drilling workers, the oil workers' lesions associated with the noise and vibration can be found out, the occupational hazards of noise and vibration can be summarized, the major occupational hazards of oilfield drilling workers can be prevented and controlled, occupational diseases can be accordingly reduced, staff's quality of occupational life can be greatly improved, scientific basis for the oil companies to inspect workers' occupational health and control the occupational diseases can be well provided. Besides, through symptoms' contrast with a non-contact crowd, the influence of noise and vibration on the health status of workers can be summarized.Materials and MethodsOver 600 men who works more than one year are chosen asthe observer group, with the average length of service (13±5) a, aged 20-46 years, with an average age (30±5 years); besides, 300 healthy male of logistics company who don't contact the drilling noise and vibration are chosen as control group, whose seniority, age is similar to observation group. Two groups of workers are checked in accordance with the occupational health inspection projects in the "Law for the Prevention of occupational diseases", such as MIDIMAPE-622 pure-tone audiometry meter, peripheral circulatory function inspection, peripheral sensory nerve examination and inspection of function in hand. The two groups are compared for statistical analysis, the damage of noise and vibration on the human body is discussed.ResultsThere were significant differences in symptoms of observer group compared with the control group, such as tinnitus, hearing loss, high blood pressure, heart rate and ECG abnormalities, white finger, hand Ma, hand pain, hand bulging, sweating, cold and allergy. Between the individual, each have more significant differences. The control group's hearing loss is mainly high frequency hearing loss, and intensity is closely related with the contact time, intensity of noise. Normal vessels of observer group in hand is less than the control group, but more than the control group in abnormal vessels, and with significant differences in the peripheral sensory nerve between two group, and there is relatively close relationship between peripheral nerve function damage and quantity of the vibration.Conclusion 1. High-frequency hearing loss was significantly higher than the phrase frequency hearing loss detected, and with the increase in noise intensity and length of service, the detection rate become higher, especially 15 years of service, the rate of the high-frequency hearing loss is more than 50%, suggesting that future work should focus on these people as objects custody. Phrase frequency hearing loss in 1-5 years is incremental as the increase in the length of service, after five years it begin to decline, which maybe workers found that the frequency hearing loss is increased with the length of service, take protection measures and be out of the post. The survey found no case of noisy deafness.2. Long-term exposure to noise can raise the drilling workers' blood pressure; the rate of heart rate abnormalities, the rates of abnormal electrocardiogram bradycardia, tachycardia, ST-T-wave abnormalities in the observation group were all higher than control group, the difference has statistically significance; and the difference of arrhythmia, occasional extra block abnormal rate of the two groups had no statistically significance.3. Neurasthenia of drilling workers is general in the survey, especially dizziness headache, insomnia dream, palpitations symptoms, compared with the control group, it shows significant difference. But compared with the control group, the detection rate of acid burning, abdominal pain, P> 0. 05, have no significant difference. It may be related to the survey samples, which were all males. Stomach symptoms also have relevance with smoking and drinking.4. The incidence rate of vibration white finger is 23. 57%, lower than reported, which may be related to the mechanization improvements of the drilling operations and the decrease of vibration energy-related hazards. The symptoms such as white finger, hand bulging, trembling hands allergy in the control group, did not appear, which means the indicators is more meaningful in the diagnosis of the vibration disease.5. Local vibration caused mostly the damage to peripheral nerve function and the peripheral circulatory function, normal capillaries in vibration group was significantly lower than the control group, while abnormal capillary is significantly higher, through comparison, there is significant difference. By depth examination of left middle finger, ring finger in vibration group, compared with the control group, there was no significant difference; right middle finger, ring finger in vibration group, compared with the control group, there was a significant difference. The workers who contact local vibration have mainly the symptoms in hand and neurasthenic syndrome.Discussion1. Our drilling Company, after several technical transformation, drilling workers' working environment has been considerably improved. The on-site survey, on the drilling productive workplace, equivalent continuous noise 8h is A sound level (Leq): 61. 7dB (A)~105.9 dB (A), there are still 68 points' noise intensity operations over 8 hours 85dB (A) health standards limits in "Criteria-2002", the nature of noise is middle-high frequency, the rate over standard is 30.36%. The noise beside diesel is up 105.9 dB (A), more than 85 serious dB (A) national health standards, but drilling workers do not wear ear plugs or ear defenders when they do the operation. Therefore, intensifying enterprise occupational health regulation, strengthening occupational health education, vocational periodic health examination, paying attention to harmful noise, can not be ignored.2. The survey results, both subjective symptoms and objective function checks, indicate that drilling noise and vibration health do a certain extent of damage to workers, so do the domestic WANG Longyi's report. The survey showed that the subjective symptoms in observation group is general, 105 of 600 drilling conscious people have tinnitus, the positive rate was 17. 5%, significantly higher than the control group, and the differences were significant, which indicates the tinnitus is the major clinical manifestations that noise impacts the auditory system. The rate of hearing loss of drilling workers' is increasing as the increase in quantity of noise and the time of access service, and mainly high-frequency hearing loss-based, detection rate of high-frequency hearing loss is significantly higher than that the phrase frequency hearing loss and during the high frequency, with increasing frequency, the number of hearing loss grows, hearing loss rate increased highest to 4kHz and 6kHz, hearing curve in 4kHz was V-shaped trough, and the report concluded unanimously. In this survey, the increase in blood pressure, abnormal electrocardiogram, neurasthenic syndrome in the observation group compared with the control group, there were significant differences, and the investigation on Liu Xiumei is consistent. Therefore, the drilling operations workers who are exposed to noise, severe hearing loss and hypertension should be considered as a taboo , out of the working environment so as to avoid further aggravating damage to the body. As for workers who contact with local vibration in the drilling, these positive rate, such as white finger, hand Ma, a painful peripheral circulatory function damage, compared with the control group, were statistically significant; from the survey, a phenomenon can be seen in the local vibration of occupational hazards, the damage on function of peripheral nerve and the damage on peripheral circulatory function is auxiliary. In 10 to 15 years' service, the high-frequency hearing loss detection rate is nearly 50%, suggesting that future work should focus on these people as objects custody, the workers serving 15-year decline in the rate of high frequency hearing loss, probably because of the transfer of jobs and the implementation of protective measures.3. Through the survey, a wrinkled check indicates normal microcirculation of fingers in vibration group are significantly lower than the control group, while abnormalities vascular are significantly higher, and Wang Lin described consistently. Cold and rewarming test which is more than 30 minutes expose that observation group compared with the control group, there was no significant difference; Group of white finger with vibration and white finger with no vibration group also no significant difference, which could be attributed to one fact, the accommodativeness of finger temperature is not only related to blood flow, but also affected by many factors, such as smoking, drinking and strenuous exercise, as the reference factors is more difficult to control, particularly smoking and strenuous exercise in the survey did not be controlled; the problem in the future be subject to further study. It can be seen from the analysis on the relative risk, the damage on workers' fingertip two-points distinguish is worse than the depth feeling and pain feeling, and the right hand is worse than the left hand, it may be related to the habit that workers usually use their right hand at work.
