Hospital is key body in the healthcare industry chain. It is the core of the relationship among patients, doctors, hospitals, and governments. Hospitals are usually reimbursed in three ways: the government subsidies, charge for service and revenue from pharmaceuticals. And pharmaceuticals income is the main part that hospitals live on. Hospitals can get little government subsidies. And the charge for service is not enough. Hospitals have to rely on pharmaceuticals income. They sell far more than enough pharmaceuticals. Patients find that healthcare expenditure is too high to go to hospitals. It is important to set up a better hospital compensation mechanism. But how do we build a better hospital compensation mechanism? This thesis puts focus on the developed counties that has a long time on social security. This thesis tries to find the way how they reimburse themselves, and that is there any common point on them.This thesis analyzes the reimbursement system in an economic point of view. This thesis is composed of three parts. The first part illustrates the situation of Shanghai's basic health service system, the key concepts, the relative theories and why this thesis is chosen. In part one, this thesis analyzes the policies about hospital reimbursement system. But these policies can not solve the problem. First of all, physicians can not get a reasonable service fee, when providing service. Second, hospitals sell pharmaceuticals to compensate themselves.In part two, this thesis makes a comparison of three developed countries: the USA, Germany and Japan. In part two, this thesis use data of OECD to analyze the impact of hospital reimbursement system of the three countries.At the end of part three, a conclusion is drawn. First, the mechanism of how the hospitals get revenue should be modified reasonably. What the doctors get should be related with the cost. Second, hospitals should not sell pharmaceuticals. Third, government should give more subsidies to hospitals. After three suggestions, this thesis analyzes what the stake-holders get if the three recommendations are implemented. There is no lose of hospitals, the valuation of physicians is re-discovered. The patients acquire cheaper healthcare. And a bright future shows a road to the pharmaceutical sectors.This thesis is based on careful study of demonstration information and a lot of objective data, so it has certain reference value theoretically and practically. I hope that it can be helpful for the reform of Shanghai health system.
